Violation 10 CFR 50, Appendix B Criterion XIII and the Limerick Preliminary Safety Analysis Report Appendix D.4.9.1 require that measures be j established to prevent degradation of the quality of equipment below that required by the applicable equipment specifications.

Further, it is required that written instructions be prepared to ,

govern'the preservation of equipment prior to and after  !

installation. i Contrary to the above, on March 30, 1987, no written procedure existed to provide requirements for relief valve protection. The lack of adequate protection resulted in a breiten lockwire on the I blowdown adjustment ring access plugs on relief valve PSV '

Response

Relief valve PSV-51-2F0055A with the broken seal wire has been identified on a nonconformance report for re-verification of its setting. The recheck of settings will be accomplished in accordance with project procedures. The presently installed relief valves are being inventoried, protected as necessary and added to the Long Term Maintenance (LTM) Program.

To prevent recurrences of broken seal wires, the construction  !

procedure regarding installation of relief valves has been revised to require protection to be provided as part of the installation process. Also, the LTM procedure has been revised  ;

to include the relief valves, as they are installed.
