1.0 INTRODUCTION
In September 1993 or some period just prior to that an inspection by the NRC of one or more of Commonwealth Edison's plants took place. Rese plants are of G.E. Nuclear's design and or management. At the time of the NRC inspection, there were questions raised as to the current reliability of the Fike safety devices (Rupture Disc Assemblies) in service.
From the initial information received by Fike, it was unclear as to length of time the discs had been in service. It was also unclear when the shipment of the assemblies and or the discs used in the assemblies took place. The period appeared to exceeded the allowable NRC maximum replacement period of five years and Fike's recommendation for replacement of one year.
2.0 OBJECTIVE To provide a burst test, evaluation and confidence report for three safety devices (Rupture Disc Assemblies) returned from G.E.
3.0 KNOWN INFORMATION 3.1 Radiation Consultants of Mid-America, Inc. received a single box from G.E.
containing three disc assemblies.
3.2 The box housed a single zip-lock poly bag containing the three disc assemblies.
Inside the poly bag was a green print on white background tag showing that Quad Cities Station had released the three assemblies for %eaadi ional use," after having t
been Radiologically surveyed. De three nasamhlies each had a wired on Mack print on red background tag. He tags provided each assembly with an identification code.
Most information was common to each tag with the exeption of the TAG NO. and IDENT. OR SER. NO. Photo copies of all tags (front and back) are a part of this report.
3.3 RCM-A confirmed the assemblies had no radiarim containinntim. Reference the letter from RCM-A, Inc. dated 8 June 1994 (included as a portion of this report).
3.4 A Fike company truck picked up of the assemblies from RCM-A,Inc. Hey arrived at the plant on 8 June 1994. Receiving logged in the package and stored it until the day required for test and inspection (14 June 1994).
4.0.. TEST PROCEDURE Section 4.0 ofIPB NO. E-0038 provides the instructions for the test pucedmc.

1 5.0 TEST AND OBSERVATIONS A completed copy of the test results as shown in Appendix A (sheets 1 & 2) of this same IPB have been includedin this report.
De code identity of the assemblies as listed on the Appendix is as follows: column #1 is G.E.
code identity 94-24 (HCU locadon number 38-55), column #2 is G.E. code identity 94-23 3
(HCU location number 42-55) and column #3 is G.E. code identity 94-21 (HCU location number 38-59).
6.0 FINDINGS He lack ofinformation provided with the disc assemblies leaves openings to spelation and assumptions. The findings will state known information only in order to prevent unsubstantiated conclusions.
6.1 He three disc assemblies were Fike Screw Type Holders, Style %30, Assembly No.
1/2-30SM with a % NPT male thread.
6.2 he discs were the pre-bulged series with 30* angular seating arrangement. Each were coated on both sides with teflon.
6.3 He single Fike identity tab supplied with G.E. code 94-24 (HCU location number 38-
- 55) indicates the disc manufactured date to be in June or July 1968. The remaining two discs did not contain tags and therefore were not identifiable as to their manufacture date.
6.4 he Teflon coating on all three disc had de Immia=*~i from the concave surface. He small remaining segments after bunt did not show any signs of corrosion attack from coating loss.
' 6.5 He bunt pressufe for the known date of manufacture disc was 2156 psig calculated for 150' F operational use. De burst pressures for the two unknown dates of manufacture disc were 2182 psig & 2200 psig calculated for 150* F operational use.
De allowable working tolerance range at the time of manufacture for the known dated disc was 1900 psig to 2100 psig 5% at 1500 F.
6.6 Each of the disc thickness measured 0.035 inches. This was the original thickness for the known disc.
6.7 He physical appearance for the body, ring and muffled screw holddown nut although showing small to moderate rust was good for each assembly.
7.0 CONCLUSION
ne testing of only three assemblies does not allow Fike to provide any conclusions. The only statement Fike can make at this time is that all three burst within the allowable range of manufacturing.
