We are herein requesting an amendment to our facility license. Two changes are requested: 1. updating NRC recipients in the reporting requirements of paragraphs 3.D(1), 3.D(2),3.D(3) and 3.D(4); and 2. replacing " telegraph" with " facsimile" in paragraph 3.D(1).

Item i Details: Section 3.D(1-4) of the USGS facility license specifies reports that shall l be made to the Director of the Atomic Energy Commission Regional Office and/or the Director, Division of Reactor Licensing (DRL). Since the Atomic Energy Commission ceased existence in the early 1970's, this should be corrected to have reports sent to 4 the Document Control Desk, Nuclear Regulatory Commission, Washington, DC 20555. 1 In 1997, the USGS non-power reactor facility was moved from regional compliance regulation to regulation from NRC headquarters. Therefore, all references to the regional compliance office should be removed. In addition, references to reports made  !

to the Director, DRL should be changed to the Document Control Desk, Nuclear Regulatory Commission, Washington, DC 20555.

Item 2 Details: Paragraph 3.D(1) allows prompt notifications to be made by telegraph.

We request that " telegraph" be replaced by " facsimile" The license sections that are affected by this request are attached.

' (i) The licensee shall in'orm the Commission of any incident or condition releting to the operation of the reactor which prevented or could have ,

prevented a nuclear system from performing its safety function as described in the Technical Specifications or in the safety analysis report.

For each such occurrence, USGS shall promptly notify by telephone or facsimile the NRC Operations Center and shall submit within ten (10) days a report in writing to the Document Control Desk, U.S. Nuclear Regulatory Commission, Washington DC 20555.

t (2) As promptly as practicable, but no later than sixty (60) days after the initial criticality of the facility, USGS shall submit a written report to the Document Control Desk, U.S. Nuclear Regulatory Commission, Washington DC 20555, describing the measured values of the operating conditions or characteristics listed below and evaluating any significant variation of a measured value from the corresponding predicted value:

(3) The licensee shall report to the Document Control Desk, U.S. Nuclear Regulatory Commission, Washington DC 20555, in writing within thirty (30) days of its occurrency any substantial variance disclosed by operation of the reactor from performance specifications contained in the I safety analysis report or in the Technical Specifications.

1 (4) The licensee shall report to the Document Control Desk, U.S. Nuclear Regulatory Commission, Washington DC 20555, in writing within thirty (30) days of its occurrence, any significant change in the transient or 4

accident analysis, as described in the safety analysis report.
