Kingsley President, Nuclear Generation Group Commonwealth Edison Company ATTN: Regulatory Services Executive Towers West lil 1400 Opus Place, Suite 500 Downers Grove, IL 60515 SUBJECT: NOTICE OF VIOLATION AND NRC INSPECTION REPORT NO. 50-254/97028(DRP); 50-265/97028(DRP) Dear Mr. Kingsley: This refers to the inspechon conducted December 24,1997, through February 10,1998, at the Quad Cities Nuclear Power Station. The purpose of the inspection was to determine whether activities authorized by the license were conducted safely and in accordance with NRC requirements. The NRC conducted routine resident inspections in this period. At the conclusion
of the inspection, the findings were discussed with those members of your staff identified in the enclosed report. During this inspection, a number of concems with cmergency diesel generator (EDG) reliability and operability were identified. These concems involved equipment problems, personnel errors, and program-related deficiencies. A common mode failure mechanism was identified with time delay relays for all three EDGs which could have affected operability of all generators . ' concunently. In the same time penod, operator error and flawed programmatic requirements resulted in an improper cooling water lineup which rendered the Unit 2 EDG inoperable for about two weeks, while the Unit 1 EDG was unavailable for maintenance. On January 5,1998, the Unit 1 EDG failed to start after suceiving a start signal, for reasons that have not yet been determined. The inspectors found other maintenance, engineering, and operations-related problems associated with the EDGs, including poor follow-up on conective actions for previous EDG failures, poor foreign material exclusion control, and design configuration control problems. Based on these and other findings of this inspection, the NRC has determined that two violations of NRC requirements occurred. The violations are cited in the enclosed Notice of Violation (Notice) and the circumstances surrounding them are described in detail in the subject inspection report. The first violation is of concem because EDG inoperability rendered both available trains of the standby gas treatment system inoperable for a time longer than allowed by Technical Specifications with Unit 1 operating at rated power. / The second violation is of concem because the plant design configuration for EDG room i ventilation as described in the Updated Final Safety Analysis Report was altered, and no t engineering evaluation was performed to justify the change. This subject was discussed extensively with Commonwealth Edison representatives during the enforcement conference on January 9,1998. No response regarding this apparent violation is required at this time. You are required to respond to this letter and should follow the instructions specified in the enclosed Notice when preparing your response. The NRC will use your response, in part, to determine whether further enforcement action h necessary to ensure compliance with regulatory requirements.
