SUBJECT:

AUGMENTED INSPECTION TEAM - DESIGN AND TEST CONTROL PROBLEMS AT HOPE CREEK GENERATING STATION

Yo'u are directed to perform a prompt inspection ~~of ass ~ociated ~ equipment problems and test anomalies evidenced during the loss of Offsite Power (LOP) .

testing at Hope Creek on September 11 and 19, 1986. The inspection shall be in accordance with NRC Appendix 0513 Part III and the additional instructions in this memorandum. DRP is assigned the lead for the activity and E. Wenzinger is designated the team leader. DRS is assigned responsibility of providing specialist technical support. The team will also include the provision for participation by specialist inspectors from IE and NRR.

OBJECTIVES The general objectives of the AIT are to: .

a. Conduct a timely, thorough and systematic inspection related to the test anomalies that occurred at Hope Creek Generating Station during the LOP testing on September 11 and 19,1986.
b. Assess the safety significan'ce of the events and communicate to Regional and Headquarters management the facts and safety concerns related to the test problems identified.
c. Collect, analyze, and document all relevant data and factual information sufficient to determine the causes, conditions, and circumstances pert. tining to the events.

SCOPE OF THE INSPECTION f

The AIT response should identify and document the relevant facts and determine the probable causes and should be limited to the issues directly related to the anomalous system responses to the test stimulus.

Specifically the AIT should:

a. Develop a list of the test anomalies associated with the LOP tests.
b. Assess and evaluate the significance of the identified anomalies.
c. Assess and evaluate why the preoperational test program and subsequent surveillance testing did not identify these problems prior to the LOP tests through a detailed review of the following:

' s Reactor Building HVAC, Station Air / Control Air System, and Emergency Diesel Generator Controls.

d. Determine if, and to what extent, inadequate design controls, QA oversight and training may have contributed to the test anomalies.
e. Analyze the licensee's actions pertaining'to the identified deficiencies during and subsequent to the LOP testing on September 11 and 19, 1986.

f.

Assess the extent to which the root causes of the test anomalies may impact other plant systems not addressed in the LOP testing.

SCHEDULE The AIT shall be dispatched to the Hope Creek Generating Station not later than September 24, 1986, and shall remain there as long as necessary to accomplish the objectives of this inspection. It is expected that this will take no longer than five working days.

The final report should be submitted to me not later than October 24, 1986.
