Summary During an ongoing inspection of Category I Motor-0perated Valves (MOVs), control wiring which appears to lack appropriate qualification was discovered in five (5) MOV actuators.

Immediate Action Taken Nonconformance and Disposition Reports were issued to inspect all Category I Limitorque actuators.
DLC letter 2NRC-6-027 (dated 3/20/86) provided initial written notifi-cation to Region I.
DLC letter 2NRC-6-049, (dated May 15, 1986) pro-vided additional details.

Description of Deficiency As of October 9,1986, 164 of the 187 (88%) Category I Limitorque actuators had been inspected.
Of those actuators which had been in-spected,110 (66%) were found to contain unqualified wire.
The un-qualified wiring included wiring supplied by ITT, and Kalas Manufac-turing. Co., and Raychem.
The Raychem Flantrol was determined to be qualified for outside-containment applications; however,. BVPS-2 has not specified this wire &s qualified for use in actuators located inside containment.
The subject wiring is used in the control of the actuators (torque switch. and limit switch circuits).

Analysis of Safety Implication The affected valves are located in various safety-related systems.
Preliminary review indicates that failure of these wires could ad-versely affect the abilty of associated valves to perform their intended safety functions.

Corrective Action To Resolve the Deficiency Limitorque was requested to provide the basis for the qualification and use of the subject wiring.
Limitorque's responses provided some additional information; however, the BVPS-2 project detennined that use of Rockbestos Firewall III or Raychem Flametrol (outside contain-ment only) provided the most expedient path to achieving acceptable qualification.
Of the 110 actuators with unapproved or uncertain control wire quali-fications, the control wires have been replaced with qualified wire on 96 (87%) of these actuators.
It is anticipated that repl acement of wiring in the additionEl fourteen (14) actuators with identified wire qualification deficien-cies and in any of the twenty-three (23) yet-to-be-inspected actuators where defiencies are identified will be completed prior to fuel load.
