Dear Chairman Zech:

The New Jersey Board of Public Utilities was notified of an unusal event that was declared at the Hope Creek facility on August 8, 1986, that resulted in a shutdown of the plant due to an error found in a safety pressure relief sensing system. It is our understanding that during preliminary tests, technicians discovered that pressue relief sensing lines, which are part of the plant's safety system designed to sense and release high pressure within the containment b:lilding, were installed incorrectly. Apparently, the lines were reversed in accordance with the original design print that a draftsman had incorrectly drawn and the error in the design print was not discovered until after the facility was constructed and power ascension tests initiated.

The New Jersey Board of Public Utilities is concerned that there may exist additional undiscovered problems similar to the above that involve other critical areas of the plant; and this Board is concerned that the discovery of similar conditions after the plant has been declared commercial and included in the utility's rate base may lead to extended outages where the plant cannot provide power.

We recognize that the responsibility for the proper construction of the plant so that it meets established safety standards lies with the NRC and in that regard, we would appreciate being advised of those actions being taken by the Nuclear Regulatory Commission to insure that other similar conditions do not exist.
