Indian Point Post-Exercise Assessment Modification Attached is a mmorandum from Federal Emergency Management Agency (FEMA) Region II which identifies an additional deficiency in the June 4, 1986 full-scale exercise at the Indian Point Nuclear Power Station, in Buchanan, New York. On October 6,1986, we sent you the Post-Exercise Assessment identifying 6 deficiencies in that exercise.
Inadvertently, another problem, although correctly described on page 95 of the text of the report, was inserted in the wrong category in the sumary on page 98.
'Ihe problem concerned various training and equipment inadequacies in the Personnel Monitoring Center in Orange County. 'Ihis was listed as an Area Requiring Corrective Action but should have been listed as a Deficiency on page 98. We have informed the State of New York of this administrative error. It will be among the items addressed by the remedial actions to be completed by the State.

Additional Deficiency to Post Exercise Assessment Report for Indian Point This is to advise you that on October 6, 1986, in the process of preparing for a remedial exercise for Indian Point, the Regional REP staff identified a problem area in the report that had not been categorized as a deficiency.
The attached letter was forwarded to the New York State Disaster Preparedness Commission Chairman advising him of this matter.
