This Potentially Reportable Significant Deficiency Report is being submitted pursuant to the requirements of 10CFR50.55(e).

Summary Surveillance by the NRC noted two of-the six required 1/2 inch A-325 bolts were missing from each of the Emergency Diesel Generator (EDG) room air f an supports (2HVD*FN271A&B), and that the required tack welds on the
' A-194 nuts were not performed as required by the structural drawings.

Immediate Action Taken A Nonconformance and Disposition Report was. issued to add the missing bolts and tack weld the nuts.
Mr. L. M. Rabenau, Lead Compliance Engineer, Beaver Valley Power Station Unit No. 2,- notified Mr. Lowell Tripp of NRC Region 1 of this concern on February 12, 1987.

Disposition of Deficiency During surveillance by the NRC Resident inspector, it was noted that two of -the six 1/2 inch A-325 bolts required to attach f an 2HVD*FN271A to its structur al support were missing.
The same condition was also found to exist on f an 2HVD*FN2718. A records review for this system indicated that the system had been turned over for testing, and that the fans had been inspected and accepted by DLC/ Site Quality Control for location and installation within the duct run.
An installation inspection of the equipment had been performed, although inspection for bolting of the supports had not been conducted.
A verification inspection, performed by DLC/SQC, of the above condition noted that the tack welds on the A-194 nuts had not been performed as required by structoral drawing RS-29-C.

,A,nalysis of Safety impligation The Quality Control Piocedure for this activity was incomplete in that inspection Plan, IP-7.4.1, " Inspection of QA Category I and Other Seismic Heating, Ventilation and Air Conditioning Systems Installation,"
did not clear ly and explicitly identify the inspector's responsibilities with regard to the inspection of bolting for HVAC equipment supports, in par ticu l ar, neither the requirements for bolting, nor location of such with respect to supports, was addressed.

Corrective Action to Resolve the Deficiency Nonconformance and Dispositon Report 34506 was dispositioned to install the missing bolts and tack weld the nuts on EDG room air fans 2HVD*FN271A
& B.
DLC/SQC issued an immediate Change to IP-7.4.1 on February 4,1987, to address the inspection of supports.
