SUMMARY

Through the review of operating experience, AEOD has continued to monitor the occurrence of events involving the wrong unit, the wrong train, or the  :

wrong component. This technical review provides a supplementary list of 28 additional events, 26 of which occurred since June 30, 1986, and tables of the annual number of events and the rate of occurrence in terms of events per reactor-year over the 1981 to 1986 time period.

DISCUSSION On August 15, 1986, the NRC Division of Human Factors Technology personnel met with NUMARC representatives to discuss NUREG-1192, "An Investigation of Wrong Unit or Wrong Train Events" and were told of industry efforts (procedure  ;

improvements, labelling improvements) to reduce the occurrence of such types of events. DHFT suggested to AEOD in a memo following the DHFT/NUMARC meeting that a continuing effort to identify wrong unit / wrong train / wrong component i

events was desirable, and that the focus of future updates identify the rate of occurrence of wrong unit, train, or component events, rather than simply the number of such events.**

This review provides the most recent tabulation of wrong unit / train / component events reported to NRC in licensee event reports (LER's) and covers the time period from 1981 through 1986. It represents an ongoing assessment of industry efforts to reduce the occurrence of these types of events.
