This refers to the inspection conducted by Messrs. R. Correia, P. Milano, K.C. Leu, and D. Ambrosek of this office on June 23-27, 1986, of your facility at Lynchburg, Virginia and to the discussions of our findings with Mr. C. W. Pryor and members of your staff at the conclusion of the inspection.

The purpose of this inspection was to review the status of previous inspection findings and to review Babcock & Wilcox's (B&W) activities related to modifi-cation of safety-related components, systems and/or structures for Arkansas Nuclear One (AN0) and Oconee Units 1, 2, & 3 and the receipt and evaluation of deficiencies potentially reportable under 10 CFR Part 21. However, several of the findings discussed below may be applicable to all B&W designed facilities.

During the inspection it was found that the implementation of your Quality Assurance (QA) program failed to meet certain NRC requirements. These findings relate to (1) the review of potential safety concern 17-83, Overceoling Events dt Low Reactor Power, and demonstrate weaknesses in safety-related engineering activities and a failure to ensure that known deficiencies are adequately evaluated; and (2) a review of calculations supporting the ANO modification Radcal Gamma Thermometer, demonstrate weaknesses in the implementation of your quality assurance program in the area of design control. The specific findings and references to the pertinent requirements are identified in the enclosures to this letter.

Please provide us within 30 days from the date of this letter a written state-ment containing:

(1) a description of steps that have been or will be taken to correct these items; (2) a description of steps that have been or will be taken to prevent recurrence; and (3) the dates your corrective actions and preventive measures were or will be completed. Consideration may be given to extending your response time for good cause shown.
