As a result of the inspection conducted on August 13, 1986, and in accordance i

with the NRC Enforcement Policy (10 CFR 2, Appendix C), the following v.iolations l

were identified:

A.

Condition 11 of License No. SNM-1147 requires that refresher training in radiation protection be provided annually to each individual using radioactive materials and that the training be appropriate to the type of Itcensed material used and for the individual using the materials.

Contrary to the above, as of August 13, 1986, refreshar training had not been provided to individuals using licensed material during calendar year 1985 or to date in 1986.

This is a Severity Level IV violation.

B.

Condition 9 of License No. SNM-1147 requires that licensed material be possessed and used in accordance with the statements, representations, and procedures contained in an application dated March 18, 1981.

Page 5 of this application, entitled, " Administration and Organization", requires that the Radiation Safety Committee meet

.i semi-annually; and Section C of the Radiation Safety Manual included with this application requires that records be maintained of Committee actions.

Page 8, Item L of this application requires that physical inventories of all sealed sources be conducted quarterly and utilization logs be maintained.

Contrary to the above, as of August 13, 1986, quarterly inventories were not conducted. Specifically, during 1984 and to date in 1986 one inventory was conducted; during 1985 no inventory was cunducted.

Contrary to' the above, as at August 13. 1586; leak tests were not conducted quarterly. Spaifically, during 1984 and 1985 leak tests were conducted biannually and in 1986 leak tests were conducted once in July.
