NOTICE OF VIOLATION Commonwealth Edison Company Docket Nos. 50-454; 50-455 As a result of the inspection conducted on November 1 - December 1, 1986 and in accordance with the " General Policy and Procedures for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1986), the following violation was identified:
Technical Specification 3.3.9.6 requires that radioactive liquid effluent monitoring instrumentation channels be operable at all times or else take the actions shown in Table 3.3-12.
Table 3.3-12, Instrument 2.a(1)(a), requires that 1RE-PR002, " Essential Service Water, Reactor Containment Fan Coolers IA and IC Outlet Monitor," be operable or else follow Action Statement 32.
Action Statement 32 requires that with monitor 1RE-PR002 inoperable, effluent releases via this pathway may continue for up to 30 days provided that, at least once per 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />, grab samples are collected and analyzed.
Contrary to the above, for 48.5 hours5.787037e-5 days <br />0.00139 hours <br />8.267196e-6 weeks <br />1.9025e-6 months <br />, from 0020 on October 23, 1986 to 0052 on October 25, 1986, while in Mode 1, radioactive liquid effluent monitoring instrumentation channel IRE-PR002 was inoperable and the required grab samples were not obtained. Grab samples obtained for other reasons were not analyzed in the time frame required to meet this action statement.
This is a Severity Level IV violation (Supplement I). (454/86047-02(DRP))
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each violation:
(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further violations; and (3) the date when full compliance will be achieved.
Consideration may be given to extending your response time for good cause shown.
