Refeseece (a) ir.dicated that certain activities appeared to be in noncompliance with NRC requirements. . documentation of EQ equipment to be maintained in an auditable form to permit verification that the equipment is qualified for its application and meets its performance requirements during an accident. ,

Contrary to the above, the environmental qualification (EQ) Binder LS029

~

for WEED thermocouples did not include a justification for allowing a WEED thermocouple to be installed without EQ maintenance activities being implemented. During this review, the NRC inspectors identified one WEED thermocouple 2TE-CM058, whose housing had been opened for maintenance, but '

for which appropriate maintenance activities specified in the EQ binder had not been implemented. The licensee stated that their failure to perform the maintenance activities in the EQ binders did not affect the environmental qualification of the WEED thermocouple based on the testing of a PYCO thermocouple; however, the inspectors found no similarity analysis for the PYCO and WEED thermocouples in EQ binder LS029.

CORRECTIVE ACTION TAKEN AND RESULTS ACHIEVED The Weed thermocouple immediately had its EQ maintenance performed (WR L65410) as stated by the EQ Binder.

An engineering analysis (CQD 034822) was performed which provided justification demonstrating the maintenance items omitted in the work request were not critical to the qualification of the instrument.

CORRECTIVE ACTION TAKEN TO AVOID FURTHER NONCOMPLIANCE Maintenance history of all cr.vironmentally qualified Weed thermocouples was reviewed for missed EQ maintenance. It was determined that the thermocouple uncovered during the audit was the only one having maintenance performed on it without addressing the EQ maintenance items.

The stations work request procedure (LAP-1300-1) now requires a documented Technical Staff EQ review of work instructions prior to their performance in an EQ work package. This EQ review ensures all applicable 3Q requirements are addressed in the work package. This added review was not in affect at the time of the work package which omitted the EQ maintenance on the Weed instrument.

DATE OF FULL COMPLIANCE Pull compliance was achieved on March 6, 1987.
