SECTION 1 PURPOSE The purpose of this report is to present a description and analysis of the April 1986 Type A Reactor Containment Building Integrated Leakage Rate Test (ILRT), summary of the Types B and C Local Leakage Rate Tests (LLRTs), Air Filtration System Test Results, and Recirculation (RHR) System Tests conducted by the Connecticut Yankee Atomic Power Company and Northeast Utilities Service Company. A summary of CYAPCO's Corrective Action Plan for Types B and C penetrations is included. This report is submitted, as required, by 10CFR50, Appendix J Para-graph V.B. (Reference 1) and CY Technical Specifications.

SECTION 2

SUMMARY

2.1 Type A Test Pressurization for the ILRT was started approximately 2259 hours on April 2, 1986. The compressors were secured at approximately 0625 hours on April 3, 1986, with a peak pressure of 30.8 psia. At 1100 hours on April 3,1986, gross leakage (> 0.4 percent per day) was detected by the ILRT instrumentation. Investigation of penetration areas was conducted. By 1630 hours on April 3, 1986, penetrations P-3 (HPSI), P-81 (Auxiliary Feedwater), and P-65 (Air Monitoring System) were isolated and closed. A four-hour stabilization period following isolation of the valves commenced. During this time, leakage was monitored and trended. Computer analysis indicated no change in the leakage rate occurred. By 2013 hours on April 3, 1986,.the leakage was traced to an open service water vent valve in containment (going to containment fan cooler no. #2). Valve SW-V-266 was closed and then blank flanged. The measured leakage decreased significantly over the next few hours.

Wwf' At 2200 hours on April 3, 1986, ILRT instrumentation indicated that the containment air temperature was increasing significantly (> 0.85'F/HR) . This trend continued on into the early hours of April 4, 1986. Analysis indicated that the rise in temperature could potentially

     " thermally mask" the actual integre.ted leakage rate. A methodical review of all systems and thermodynamic parameters, that could account for this effect, was conducted.

At 1445 hours on April 4, 1986, service water valve SW-V-264 (going to fan cooler no. #1) was cracked open. This action proved effective. By 1610 hours, the temperature rise was less than 0.2*F/ hour. Since the service water system operates under DBA LOCA conditions, it was determined that reintroduction of cooling flow to a fan cooler was acceptable (Reference 1). At this point, the test was restarted and a four-hour temperature stabilization period commenced. At 2036 hours on April 4, 1986, the 24-hour L measurement start-tm ed. On-line computer analysis trending indicated acceptable results and no further containment problems were encountered. The test equipment performed without problems during the Type A test. During the 24-hour L , measurement, no reweighting of ILRT instru-ments was required. I

_4_ At 2039 hours on April 5, 1986, the L , portion of the test was successfully completed. The LSF and 95 percent. upper confidence limit test results indicated that the containment leakage was essentially zero. A superimposed leakage verification test was successfully completed from 1009 hours to 1409 hours on April 6, 1986, and containment depressurization followed. 2.1.1 Conclusion The April 1986 CY ILRT Type A test is considered a successful test in that it demonstrated essentially zero leakage across the containment boundary. The measured leakage rates are well within the plant's maximum allowable leakage rate. 2.2 Local Leakage Rate Tests (Types B and C) The results of Local Leak Rate Tests (LLRTs) of containment isola-tion valves and other containment penetrations are documented per Reference 10. In accordance with 10CFR50, Appendix J, Paragraph V.B., data for the LLRTs are summarized.in Section 4 of this report.

SECTION 3 TYPE A TEST 3.1 Edited Log of Events This log was edited from the information contained in the Official Log of Events (per Reference 2). i April 2, 1986 1 0700 - Data acquisition system placed in operation. 2355 - Containment smoke detector alarms. Operations prepares to make containment entry. April 3, 1986 0005 - Operations enters containment. Visual inspection indi-cates alarm false (no smoke or fire inside containment). Operations exits at 0040. 0043 - MCB containment high pressure alarm occurs at 4.4 psig. 0625 - Secured containment pressurization. A test pressure (P ) of 30.8 psia was attained. 0650 - Commenced containment temperature stabilization period. 1100 - Gross leakage (p=0.4 percent per day) detected by ILRT instrumentation. Commenced extensive investigations of all penetration areas. 1630 - Penetrations P-3 (HPSI), P-81 (Auxiliary Feedwater), and

             ?-65 (Air Monitoring System) isolated and closed. Test reinitialized and four-hour temperature stabilization period started.

Commenced reinspection of pene-tration areas. 2013 - Leakage traced to service water vent valve (to CAR fan cooler no. 2) SW-V-266. Valve closed. Residual leakage detected. 2200 - Blank flange installed on SW-V-266. -ILRT leakage begins to decrease. Commenced four-hour temperature stabiliza-tion period. 2231 - Computer trending indicates a rise in containment air temperature of 0.85 to 0.9'F/ hour. Trend continues into early morning hours of April 4, 1986. Computer analysis indicates the rise in air temperature could potentially

                             " thermally mask" the actual ILRT leakage rate. A method-ical engineering review of systems thermodynamic behavior (to account for this trend) is conducted.

April 4, 1986 1440 - It is determined that the original leak through CAR fan cooler no. 2 service water valve SW-V-266 had " depressed" the effect of the CAR fan motor heat output. When the leakage was terminated, so was the cooling flow through the cooler itself. It was determined that reintroduction l l j

1445 - Service water flow to CAR fan cooler no. 1 initiated (by cracking open SW-V-264). Containment temperature rise began to decrease. 1610 - Temperature rise calculated to be 0.1 to 0.2*F/ hour. Test reinitialized and four-hour temperature stabiliza-tion period started. 2036 - Temperature stabilization completed. Start of 24-hour containment leak test. Appendix G (Reference 2) satisfied. - iF April 7, 1986 Plant began depressurization of containment. 3.2 General Test Description 3.2.1 Prerequisites i In accordance with the CYAPCO ILRT procedure, SUR 5.7-108 (Reference 2), the following is a listing of the perti-l l nent prerequisites that were completed and documented l prior to containment pressurization: I r. l

h. All compressed gas cylinders removed from contain-ment.
i. All test instrumentation calibrated and verified functional.
j. The Data Acquisition and Analysis Computers were operational and programmed for ILRT functions.

k. Containment water levels were taken prior to the start of the Type A test.

3.2.2 Equipment and Instrumentation Pressurization of the containment was achieved by the utilization of temporary air compressors. Air was piped through an after cooler and a refrigerated air dryer. The system included adequate instrumentation and valving to maintain proper monitoring and control of the compressed air quality throughout the pressurization sequence. The total rated capacity of the pressurization system as installed was 6,000 SCFM. The various containment parameters required to calculate containment leakage during the test were monitored using instrumentation which consisted of resistance temperature detectors (RTDs), dewpoint sensors (RTD type), and an absolute pressure quartz manometer. Pertinent data for the test instrumentation is listed in Attachment 3.2A and the general locations of the temperature and moisture sensors are shown in Attachment 3.2B. A mass flowmeter was used to perform the superimposed leakage verification test. All test instrumentation was monitored by Northeast Utilities' Data Acquisition System.

3.2.3 Data Acquisition System The data acquisition system used for the CYAPCO ILRT was provided by Northeast Utilities Service Company. The data acquisition system monitored the following instrumentation: y

The instantaneous instrument readings and averages were both recorded.

3.2.4 Data Resolution System After the appropriate data had been acquired, utilizing the HP 3497A, the parameters were input into Northeast

For the CYAPC0 ILRT, the absolute method of mass point analysis was used to determine the leakage rate. Absolute Method of Mass Point Analysis The absolute method of mass point analysis consists of calculating the air mass within the containment struc-ture, over the test period, using pressure, temperature, and dewpoint measurements made during the ILRT. 1 1 The leakage rate is then determined by plotting the air mass as a function of time using a least-squares fit to determine the slope, A = dM/dT. The leakage rate is expressed as a percentage of the air mass lost in 24 hours or symbolically: Leakage Rate = (A/B) (-2400) (Eq. 2) Where A is the slope of the least-squares curve and B is the y-intercept, the sign convention is such that the leakage out of the containment is positive and the units are in percent / day. The air mass is calculated and the results are correlated as a function of time by means of a least-squares curve fit of the form: M = At + B (Eq. 3) The slope A and the y-intercept B are then used in Equation 2 to determine the leakage rate. A 95 percent confidence interval is calculated using a student's T distribution.

Section 4 and Attach-ment 4A documents the plant's corrective action and retest efforts to reduce Type B and C leakage rates. CY did not pass the "as-found" Type A test.

3.3.2.10 Discussion of Test Results The results listed in Section 3.3.2.7.1 demon-strate the "as-left" leak tightness of the CY containment boundary. The ILRT measured leakage rates (L tm LSF and UCL) satisfied all test (Reference 2) acceptance criteria. The results listed in Section 3.3.2.9.1 show that the "as-found" condition of the CY con-tainment did not meet the requirements of the test acceptance criteria (Reference 2) due to leakage through Type C penetrations. Rework and retest efforts decreased the Type C leakage significantly, as indicated in Section 4 of this report. Graph 1 (Attachment 3.3F) trends the ILRT leakage rate over time (24 hours). The L tm curves (LSF and UCL) fall under the test acceptance criteria (0.0827 % per day). Graph 2 (Attachment 3.3G) trends the average containment air temperature over time (24 hours). The temperature curve is stable

and increasing at a very low rate ( 4 0.15*F per hour). Graph 3 (Attachment 3.3H) trends the contain-ment air pressure over time (24 hours). The pressure curve is stable and increasing at a very low rate ( 6 0.0087 psia per hour). The increase in pressure is due to the effect of the temperature rise, in accordance with the ideal gas law. Graph 4 (Attachment 3.31) trends the containment vapor pressure over time (24 hours). The vapor pressure behavior is acceptable. Review of dewpoint temperature listed in Attachment 3.3B indicates trending is satisfactory. Finally, Graph 5 (Attachment 3.3J) trends the containment air mass over time (24 hours). The curve indicates a nearly constant air mass (approximately between 345,400 to 345,500 LBM) was present during the ILRT. The slope depicted in Graph I (Attachment 3.3F) also indicates a nearly constant air mass was present. The mass plot is satisfactory.

3.3.2.11 Final Conclusion The April 1986 CY "as-left" ILRT was successful and demonstrated the leak tightness of the containment building's boundaries. The plant passed the requirements of a Type A test (References 1 and 2).

SECTIONS 4 THROUGH 6 Section 4.0 summarizes the local leakage rate tests (LLRTs) data that was established to support the overall containment leakage testing program. These LLRTs were performed per the requirements of the CY LLRT surveillance program. The acceptance criteria for Types B and C testing is in accordance with 10CFR50, Appendix J, as amended by existing exemptions. Individual penetration leakage rates for all containment isolation valves and Icakage barriers subject to Types B and C tests are listed in Section 4.0. 4.1 LOCAL LEAKAGE RATE TEST CORRECTIVE ACTION PLAN AND JULY 1986 LLRT RESULTS The following is a summary of the corrective action performed on the containment penetrations with significant as-found leak rates. Leak rates in excess of .02200%/ day at 40 psig are considered significant with respect to the combined local leak rate. The corrective action for containment penetrations with significant leak rates was reviewed for additional action. The additional action consisted of mid-cycle testing and modification. Mid-cycle testing was considered if it was not felt that the failure mechanism was corrected. Modifications are planned for penetrations that had repetitive failure rates. During July 1986, the plant was shut down for steam generator repairs. Many of the penetrations considered for mid-cycle testing were tested at this time. The details are provided under each penetration. The plant had been operating for approximately two months from the latest refueling outage. The results of the mid-cycle tests are attached. P-3 High Pressure Safety Injection The penetration has four check valves in parallel for containment isolation. The initial as-found leak test was satisfactory. Leakage was detected past SI-CV-862B during the

ILRT. SI-CV-862B was not seating properly due to hinge wear. The hinge was built up with_ weld metal and the seating' angle redrilled. The retest was satisfactory. No additional action was warranted. P-7 Reactor Coolant Pump Seal Water Return

           'The penetration has three containment isolation valves in parallel. The total as-found leak rate of the penetration was
             .07011%/ day, which consists of .03249%/ day for CH-TV-334,
             .03722%/ day for CH-RV-332, and .00040%/ day for CH-CV-262.

Abrasive wear was found on the seating surface of CH-TV-334. The seat and plug were repaired. It is felt that the seat wear was related to the deterioration of the seal water return filter. This could not be confirmed. A mid-cycle leak test conducted during July shutdown indicated that CH-TV-334 was performing satisfactorily (.00006%/ day). No additional action was warranted. The initial leakage of CH-RV-332 was reported as.04955%'/ day. A bench test of the relief valve indicated that there was no valve leakage. A review of the test procedure showed that there were several test boundary valves whose leakage would be attributed to CH-RV-332. The procedure was revised to ensure that there was no leakage from the test boundaries. A retest with CH-RV-332 reinstalled was satisfactory. A mid-cycle test

during the July shutdown was also satisfactory. No additional action was warranted. P-10 Reactor Coolant System Letdown The containment isolation boundary has three containment isolation valves in parallel. Due to the piping configuration, the valves are leak tested simultaneously. The seating surfaces of LD-A0V-202, 203, and 204 had eroded. 'The cause of the erosion was an increased differential pressure across the valve due to wear of the letdown orifice (lo'cated immediately upstream of each valve). The orifices were replaced and the valves repaired, with one valve peing replaced. A retest was satisfactory. P-10 was reviewed for mid-cycle testing. Due to the large as-found leak rate, it was felt an additional test was desirable. However, since the failure mechanism was addressed, mid-cycle testing was only recommended if the plant was shut down for other reasons and plant conditions permitted testing. The penetration was not tested during July due to the short outage duration and manpower constraints. P-30 Containment Space Heating Steam Supply The containment penetration has two check valves in series. The leakage past HS-CV-295A was .20419%/ day. Signs of disc

drift and rust particles were found on the valve seat. The redundant valve, HS-CV-295, had a leak rate of .06381%/ day. The threads on the end caps that hold the disc pins in place were stripped. This allowed the discoto drift on the valve seat. The identified problems were corrected and a retest was satisfactory. Mid-cycle test during the July shutdown measured leak rates of

           .02909%/ day past HS-CV-295 and .00002%/ day past HS-CV-295A.

The leakage past HS-CV-295 was between the valve body and the seat ring. The seat ring is threaded into the valve body. The threads were cleaned and a bevel was put cn the top of the seat ring threads to improve the surface contact. The retest was satisfactory. P-30 has been selected for modification during the 1987 refueling outage. The specific cause of the valve leakage has always been identified and corrected. However, the valves have begun to show an increasing trend in leakage over the past three refueling outages. It is believed that the root cause of the leakage is valve age and valve design. The modification will address these concerns. P-38 Component Cooling Water to Reactor Coolant Pump Thermal Barrier

P-60 Component Cooling Water to Neutron Shield Tank Cooler These two penetrations are addressed together as they have similar configurations and failure causes. Each penetration has a single check valve for containment isolation purposes. The check valves have a history of repetitively high leak rates. Previous action consisted of changing the valves from swing checks to lift checks during and before the 1980 outage. A slip stream filter was installed in the component cooling water system between the 1983 and 1984 outages. The filter was installed to remove sand from the system. Disassembly of the check valve _showed that sand and silt was causing the piston to stick open. The valve internals were cleaned. A new slip stream filter, with a greater capacity to remove sand and silt, was installed in the component cooling system. A series of leak tests were conducted after the filter was placed in service. The check valves were not seating. The isolation valves for the RCP thermal barrier

                                  ,    loop and neutron-shield tank cooler loop were cycled in an l                                       attempt to remove residual sand. A subsequent series of leak tests indicated that CC-CV-721 (P-38) was functioning properly and CC-CV-885 (P-60) was sticking open.

CC-CV-885 (P-60) was replaced with a horizontal lif t check valve. It was concluded that wear of the valve internals l r

allowed system flow to cock the piston of the angle lift check valve. A subsequent series of tests indicated that the new valve was functioning properly. It was concluded that the slip stream filter was removing suspended solids from the component cooling water. However, P mid-cycle testing was considered important for P-38 and P-60. The penetrations have had repetitive leakage problems in the past. However, the corrective action addressed the root cause of the leakage. The mid-cycle tests would provide confidence in the long-term effectiveness of the corrective action. P-38 and P-60 were mid-cycle tested during the July outage. P-38 had a leak rate of .00001%/ day and P-60 had a leak rate of .00020%/ day. Initially, a large leak' rate was reported for P-38. The large leak rate was due to leakage past a test boundary valve. In addition, CC-CV-721 (P-38) was disassembled for inspection. The valve internals and seating surfaces were clean. The seating surfaces were wiped with a clean white rag. No sand or silt was on the seating surfaces. This testing provides assurance that the containment isolation valves for P-38 and P-60 will remain operable for the existing cycle.

The long-term corrective action planned for P-38 and P~60 is to modify the containment penetrations. An additional modification will minimize any sand that enters the component cooling system from the service water system cross-connection. This work is scheduled for the 1987 refueling outage. i. P-39 Containment Purge, Exhaust The penetration has a 42-inch butterfly valve, and 8-inch manual valve, and a 6-inch blind flange which are in parallel. Initially, a leak rate of .04391%/ day was measured. A blind flange is attached to the piping to form a test boundary for the leak test. The primary leak path was across the blind flange installed for a test boundary. The blind flange was reinstalled. The subsequent leak test was satisfactory. The containment boundaries in this penetration have a good history of being leak tight. 4.1.1 conclusion The corrective and preventive maintenance identified for each penetration provide adequate assurance that the containment isolation valves will function properly. Containment , penetrations with high Icak rates will be reviewed for mid-cycle testing and modifications to ensure containment integrity is maintained.

6.0 Recirculation System Tests The recirculation system test is a visual inspection for leakage of the accessible portions of the Residual Heat Removal (RHR) System located

outside of containment. The inspection is conducted daily, at-least once per shift, during plant operation. Included in the inspection are pumps, heat exchangers, valves and piping. The total leak rate must be less than 3 liters per hour (50 cc per minute).

The inspection interval included in this report is from November 1984 through December 1985. Portions of the Chemical and Volume Control System (CVCS) used for high pressure recirculation were added to the recirculation system test in August 1985. Murley U.S. Nuclear Regulatory Commission Region 1 631 Park Avenue King of Prussia, PA 19406 Gentlemen: Haddam Neck Plant Containment Operation and Testing Report The attached report is submitted in accordance with the requirements of Connecticut Yankee Technical Specifications, Section 4.4 and 10CFR50, Appendix J. This report covers the containment operation and testing from November 1984 through May 1986. Included are the results of the reactor containment building integrated leak rate test, the reactor containment building local leak rate tests, the recirculation system test and the air filtration system test. Included with the reactor containment local leak rate tests are a corrective action plan and local leak rate test results from July 1986. The corrective action plan provides the failure cause, action taken and future actions for containment penetrations which were significant contributors to the as-found local leak rate.
