Under the Freedom Of Information Act, I respectfully request the following. data and information:

1.

All names and addresses of the next of kin of the four (4) employees who died as a result of their' injuries.

2.

Name and address'of the four (4) other employees who were k

injured in the same accident.

3.

Name of manufacturer, type of piping material, heat number, and system, that burst and caused the accident.

1 If you lack this authority I respectfully request you forward this to the proper authority; or provide me with the name, address, and telephone number of the authority having jurisdicition to comply with this matter.

Please advise of any costs involved with this 1

request.

My information indicates reactor No. 2 received an operating license y

in 1973.
