Dear Mr. Thomas:

This is pursuant to 10 CFR 2.20l(a) in response to o Notice of Violation dated August 14, 1986.

The violation concerned on electrician who, while performing equipment maintenance at the north end of the Line 2 calciner inside the contamination control area of the UO2 Building, spilled uncontained visible powder which was not reported by the worker to HPT.

The lack of reporting a known or suspected contamination spread is in violation of Company procedure XN-VF-P91,001, Rev l.

Response

The job in question was the removal of calciner off-gas electrical heaters.

This work does not require the breaking of any process lines nor opening of equipment, has not involved any contamination in the past and therefore did not require the presence of an HPT nor respiratory protection. It was discovered later that a sizeable crack had developed which allowed UO2 Powder to leak out.

In the instant cose on HPT was called by a member of management, the area was put "on mosk" and the electrician was found to be contaminated and was subsequently decontaminated. A followup urine sample showed uranium above the detection level but belo'w our. first action level of 25 ugm/l and o lung count indicated that U-235 was nondetectable.

Permanent corrective action can only come about though publication of this incident and retraining.

Maintenance plans to use this incident as a topic for September safety meetings. October is the month when Exxon Nuclear normally performs annual radiation worker refresher training and we would plan to emphasize this type of incident at that time. Training will be complete by October 31, 1986.
