DOCKETS 50-266 AND 50-301 RESPONSE TO INSPECTION REPORT 50-266/86010 AND 50-301/86009 RESIDENT INSPECTOR BIMONTHLY INSPECTION This is in response to your letter of August 8, 1986, which transmitted Inspection Report 50-266/86010 and 50-301/86009.
The NRC has concluded that two violations were identified in the Inspection Report which had been previously identified by the licensee in LER 86-003 (Unit 1), LER 86-003 (Unit 2),
and LER 86-004 (Unit 1).
The first incident concerned the return to service of a new instrument bus inverter.
This evolution was performed with the aid of a " generic" procedure.
On the date of the event (June 3, 1986), a procedure was in place for starting the inverters which were original plant equipment.
The generic procedure designed for returning electrical equipment to service was consulted prior to starting the new inverter.
Thic procedure, however, did not address the specific requirement for precharging the inverter capacitors and, thus, the inverter was improperly returned to service.
By September 1, 1986, a maintenance procedure was issued for the return to service of the new instrument bus inverters.
On June 3, the evolution was performed by Operations personnel, although Maintenance personnel normally perform this function.
Details of the inverter event will be discussed with operations personnel during the training cycle of November and December 1986.
This discussion will include detailed information regarding the start-up and return of an inverter to service, even though Operations personnel would not normally perform this activity.

Licensee Event Report 86-004-01, which is being provided in a separate letter, addresses the matters raised in the Inspection Report.
