STATEMENT OF EMERGENCY CIRCUMSTANCES The Technical Specification (TS) involved in Reference (a) would, unless amended, require shutdown of one or both reactors. The conditions leading to this situation could not have been reasonably anticipated.

A series of diesel tests were undertaken af ter the approval of an interim TS change by the NRC to determine the source of CO leakage into the Jacket Coolant Water (3CW)

System.

It was anticipated that the source of the leakage would be identified and repaired within this period. However, due to the nature of the defect we were not able to identify the source of the leakage until September 18, 1986. We discovered a crack in No.12 cylinder liner of No.12 Diesel Generator (DG). We promptly determined whether the crack could safely be repaired during operations and made arrangements to assure additional back-up power supplies during the period needed for this repair. We promptly requested that the TS be further amended to permit the required repairs.

Thadani September 22,1986 Page 2 In addition to the tests and repairs we have done to date, we will examine all other cylinder liners and adapter ports in No.12 DG using the same testing techniques we used to discover the crack in No.12 liner. We will use the freon and dye penetrant tests we explained to you in earlier discussions.. Furthermore, a hydrostatic test will be done on the DG's JCW system.
