Dear Sir or Madam:
This is a request for information under the provisions of the Freedom of Information Act as amended (5 U.S.C. 552).
I request a complete and thorough search of all filing systems for all records or documents maintained by your agency concerning an alleged incident on Dec. 31, 1986, or Jan. 1,1987, at the Davis-Besse nuclear power plant, Docket 50-346.
The incident involves the alleged- behavior of Mr. Lou S torz , the plant manager, on one of the two dates listed above. It has been alleged that Mr.
Storz was at the plant while under the influence of intoxicants.
It has further been alleged that the Nuclear Regulatory Commission has prepared a report of scrae type concerning the above incident. I would like a copy of that report, along with copies of any other documents relating to the above incident.
I am making this request in my capacity as a reporter for WTOL-TV. I hope to use the requested information for a news story.
If documents are denied in part or in whcle, please specify which exemption (s) is (are) claimed for each passage or whole document denied. Please provide a complete itemized inventory and a detailed factual justification of total or partial denial of documents. Specify the number of pages in each document and the total number of pages pertaining to this request.
If there are any fees for searching for, or copying, the records I have requested, please supply the records without informing me if the fees do not exceed $50.00.
As you know, the Act permits you to reduce or waive the fees when the release of the information is considered as "primarily benefiting the public." I believe that this request fits that category and I therefore ask that you waive any fees.
