APPENDIX A NOTICE OF VIOLATION Kansas Gas and Electric Company Docket:
50-482/86-24 Wolf Creek Generating Station License:
NPF-42 During an NRC inspection conducted on October 22 and 26, 1986, two violations of NRC requirements were identified. The violations involved a failure to control combustible materials and a failure to lock valves as required.
In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions,"
10 CFR Part 2, Appendix C (1986), the violations are listed below:
A.
Failure To Control Combustible Materials In Accordance With Procedures Technical Specification (TS) 6.8.1 requires that, " Written procedures shall be established, implemented, and maintained covering... a.
The applicable procedures recommended in Appendix A of Regulatory Guide (RG) 1.33, Revision 2, February 1978," including activities for the plant fire protection program.
Administrative Procedure, ADM 13-102. Revision 5, " Control of Combustible Materials," has been established to implement these requirements.
Contrary these requirements, the NRC inspector observed the following examples of failure to follow ADM 13-102:

Contrary to Step 3.1.2, on October 22, 1986, the NRC inspector observed a wooden box (approximatt.ly 2.5 ft.X 5 ft.X 5 ft.)
made out of untreated lumber stored in train "B" engineered safety features (ESF) switchgear room (No. 3301) on the 2000 ft. level of the control building.

Contrary to Step 3.2.1, on October 26, 1986, the NRC inspector observed four wooden crates made of untreated lumber stored on the 2047 f t. level of the reactor building. The approximate sizes of the crates were 3 ft.X 3 ft.X 3 ft. (two crates),1 f t.X 2 f t.X 2 f t.,
and 1 f t.X 1 ft.X 30 ft.
Four blocks of untreated wood, each approximately 4 in.X 6 in.X 12 in., were also observed in the same location.

Contrary to Step 5.3.1, on October 26, 1986, the NRC inspector observed an unattended and unlocked, temporary flamable liquid storage cabinet (containing alcohol and acetone) on the 2051 ft, level of the reactor building.
Combustible Materials Permit 86-40 which was issued for the storage cabinet stated "... in locked flammable liquids cabinet."
This is a Severity Level IV violation.

1ADM 02-102,' Revision 12. " Control.of Locked Component Status," has been established to implement these requirements and Section 2.4 of ADM 02-102 defines " Locking Device" as any device.that prevents'a
-valve,-breaker. or components position from being changed without the
.' device iirst being manipulated (e.g., key operated locks or seals).
Contrary to the above, on October 22, 1986, the NRC. inspector observed that-the locking devices on Valves KJ-V767A and KJ-V768A required by Checklist A of CKL KJ-121.were inadequate in that the locking wire-on.
Valve KJ-V767A had slipped off the operating handle and the locking wire for. Valve KJ-V768A was so loosely installed that it could be slipped off and on. the valve operating handle without disturbing the locking seal.
This is a Severity Level IV violation.. (Supplement I.D)(482/8624-02)
Pursuant to the provisions of 10 CFR 2.201, Kansas Gas and Electric Company is hereby required to submit to this office, within 30 days of the date of the 1etter transmitting this Notice, a written statement or explanation in reply, including for'each violation:
(1) the reason for the violations if admitted,

.(2)thecorrectivestepswhichhavebeentakenandthe'resultsachieved;
-(3) the corrective steps which will be taken to avoid further violations; and
- (4)'thedatewhenfullcompliancewillbe' achieved. Where good cause is shown, consideration ~will be given to extending the. response' time.
