SEQUOYAH NUCLEAR PLANT UNITS 1 AND 2 - NRC-OIE REGION II INSPECTION REPORT NOS. 50-327/86-37 AND 50-328/86,37 - RESPONSE TO' VIOLATIONS Enclosed is our response to your letter to S. A. White which transmitted NRC Inspection Report Nos. 50-327/86-37 and 50-328/86-37 for our Sequoyah Nuclear Plant. This report cited TVA with two violations:
50-327/86-37-03, Severity Level V, the response to which is contained in Enclosure 1;.and 50-327, 328/86-37-06 which is being considered for escalated enforcement and,i therefore, no response is required at this time.

ENCLOSURE RESPONSE - NRC-OIE INSPECTION REPORT NOS. 50-327/86-37 AND 50-328/86-37 i
DR. J. NELSON GRACE'S LETTER TO S. A. WHITE DATED OCTOBER 28, 1986 l
Violation 50-327, 328/86-37-03 Technical Specification 6.11 requires that procedures for personnel radiation protection shall be prepared consistent with the requirements of 10 CFR 20 and shall be approved, maintained and adhered to for all operations involving personnel radiation exposure. These requirements are implemented by Radiological Control Instruction RCI-1, Radiological Hygiene Program, and RCI-14,. Radiation Work Permit (RWP) Program.
RCI-14 requires each individual i
employee to read and adhere to all requiremen.ts of the RWP and RWP timesheet i
1 associated with the area entered and the work performed.
RWP 86-0460-63 required that a hood be worn during work performed in a contamination zone on I
elevation 714 of the Auxiliary Building.
9 Contrary to the above, the licensee failed to adhere to the requirements of RCI-14 and RWP 86-0460-63 in that an employee was observed performing work in the contamination zone without a hood.
This is a Severity Level V violation (Supplement IV).

Admission Or Denial Of The Alleged Violation TVA admits the violation occurred as stated.

Reason For The Violation The violation occurred as a result of personnel error in that the individual failed to follow the requirements of the posted RWP.

Corrective Steps Taken And Results Achieved f
A Radiological Incident Report was writtep against the individdal,and.
disciplinary action was taken.
4.
Corrective Steps Taken To Avoid Further Violations Radiological Control Instruction RCI-l was revised to impress upon workers the importance of following good radiological work practices and adhering to the instructions specified on RWPs when working in radiological controlled areas of the plant.

Full compliance was achieved on October 10, 1986 when RCI-1, Revision 31 I
was approved and issued.
