I would request that you amend your records to show that under License Condition No. 12 the licensed material shall be used by,

[1 Vilnis Gagainis, William Rafajko and Ed Jordan.

they are no longer employed by Multigraphics.

I Under the License Condition No. 13A that requires each sealed o-source containing licensed material to be tested for leakage at
- 3, six month intervals, I am enclosing a copy of our Purchase Order

for Kits to be used for wipe tests.

- These wipe tests were submitted to Ohmart Corp. for evaluation.
Certification that the results were acceptable came with the l
six month permit on April 23, 1987.
Our next test will be made during the first week in October, 1987 and continue every six iL months without fail.
