ATTACHMENT BVPS-2 Report On IE Bulletin 85-03 Motor-Operated Valve Common Mode Failures During Plant Transients Due to Improper Switch Settings Each of the actions requested by itens "a"

through "e" of IEB 85-03 are addressed below.

This report addresses the information requested for the initial report required by item "e" of the subject bulletin.

a)

A review of the BVPS-2 Auxiliary (emergency) Feedwater Systen verified that no motor-operated valves are used in this systen.

For BVPS-2, therefore, the requirenents of IEB 85-03 apply only to the High Pressure Safety Injection Systen and those parts of the Chenical and Volume Control Systen used for the high pressure injection function.

Table 1 lists the BVPS-2 valves for which IEB 85-03 is applicable (i.e. those valves required to function to initiate high pressure injection).

For each applicable valve, Table 1 includes both the design differential pressure and the maximum differential pressure expected for nonnal or abnonnal events.

The maximum expected differential pressures were conservatively detennined by assuning a single pipe failure downstrean of the valve with pump discharge l.

pressure or volume control tank pressure, as applicable, at the-valve inlet.

l b,c) Tables 1 and 2 identify the torque, torque bypass, and limit I

switch settings.

The torque switch settings in Table 1 were based on valve manufacturer specifications.

Corrective maintenance procedure CMP-2-75M0 VATS-21E will be used to test the valve operators with the Motor Operated Valve Analysis and Test Systen (M0 VATS) equipment.

The valves will be stroke tested, operator paraneters will be measured, documented (as-found) and compared to the previously calculated values or setpoints.

Each valve is analyzed and the signatures retained for history. The valve manufacturers have provided thrust values which ensure proper operation based on design differential pressures.

The referenced procedure will adjust the torque switch settings to the calculated thrust values based on the design dif ferential pressure,and document the final (as-left) settings.

Since the MOVATS procedure is based on the thrust which would be required for the design dif ferential pressure, use of full differential pressure during this test is nat planned.

However, during preoperational and hot functional testing, proper valve operation under system flow and pressure conditions will be performed to the extent practicable.

The established open/close torque switch, torque bypass, position limit, and motor parmeters and stroke-time requirments will be included in the BVPS-2 corrective maintenance procedures.

Recording of these performance parmeters will be required for the "as found" and the "as left" conditions to ensure that the established perfonnance parmeters are achieved for the valves listed in Table 1.
