Notice of Violation Dated July 2, 1986 Concerning Nonconformance Reports Mr. A. E. Chafee's letter of July 2, 1986 provided NRC Inspection Report 86-20 and a Notice of Violation. In accordance with the instructions in your letter and the provisions of 10 CFR 2.201, attached is our response to the Notice of Violation.

Martin Docket 50-344 August 1, 1986 License NPF-1 Attachment Page 1 of 2 NOTICE OF VIOLATION 10 CFR 50 Appendix B Criterion IVI and Trojan Nuclear Plant Nuclear Quality Assurance Program, Section 15.2.3, require in part that non-conformances be identified and corrected.

Nuclear Division Procedure 600-1 " Control of Nonconforming Materials, Parts, and Components" Section 5.0, requires in part that corrective action be assigned and implemented for non-conformances.

Contrary to the above, corrective action was not assigned and implemented for Non-Conformance Report (NCR)86-024, which documented non-conformances associated with electrical conduit support for the "A" train of the hydrogen recombiner system.

This is a Severity Level IV violation (Supplement 1).

Response

Upon the determination that corrective action had not been assigned and implemented for NCR 86-024, the following corrective actions trere taken:

1. Documentation was provided and subsequently attached to NCR 86-024 to explain why it had been voided.
2. In this explanation, the voiding of NCR 86-024 was accomplished j inconsistent with the application of design standards and the National l

Electric Code specification; for that reason, a new NCR 86-121 was initiated.

3. NCR 86-121 was subsequently evaluated by NPE and dispositioned "use as is" and closed out on July 16, 1986.

To avoid further violations, the following corrective steps have been taken:

1. The Quality Assurance Manager has discussed the proper methods / reasons for voiding NCRs with the QA/QC specialist, who entered the improper voiding (as corrective action) on NCR 86-024, and the Quality Control Supervisor, who approved the recommended corrective action.
2. The Quality Control Supervisor has counseled all QC staff members responsible for NCR tracking / closing on the correct methods for voiding NCRs.
3. The Quality Assuranco Manasce, Quality Control Supervisor, and NPE Electrical Branch Manager, and the NPE Electrical Engineering Supervisor have discussed the requirements for voiding NCRs in the future.

4. The NPE Electrical Manager addressed this problem and the proper methods and reasons for recommending an NCR be voided at the NPE Electrical Branch staff meeting on July 23, 1936.

Full compliance was considered to have been achieved with these requirements by the closing of NCR 86-121 on July 16, 1986 and the discussions at the NPE Branch staff meeting on July 23, 1986.
