Damage to Beam, T. Bowman Memo 84-12-08-04 Data:
12/10/84 Control No. 84-12-10-05 T. Bowman, L.K.C. Quality Control Inspector, discovered damage to H Beam bottom of flange of the W36 x 245, Located 11'6" south of column 26 on Q Line, Reference S&L Drawing S-1312.
There is visible intermitent base metal reduction from 1/8" to 1/2" in depth, with slag inclusions caused by flange cutting. The damage extends from 11'6" south of 26 column, south into a concrete wall. The visible area of flame-cutting is 6'6" long before entering the wall, the area is also painted.
The damaged arer, can 'be viewed from floor elevation 439'.
If you have any questions concerning the above mentioned problem, Mr. T. Bowman will be made available to assist in locating the base metal reduction.
Also, an NCR has not been generated due to the nature of the problem.
If CECO PCD requires and NCR to be written, please advise the L.K.C. QC Department.

This letter is to acknowledge our receipt of your information regarding construction deficiencies involving the L. K. Comstock Company at the Comonwealth Edison Company's Braidwood Nuclear Station.
I have enclosed,
memoranda pertaining to our understanding,of your concerns.
As we discussed on September 5,1984, it would be of ime'nse value to meet with you personally and to discuss your issues.
I have arranged to fly you to the Chicago area at NRC expense, and after our September 5th discussion, I have established the following itinerary.
On the morning of September 11, 1984 you should plan to" leave your residence by your personal automobile in sufficient time to arrive at the Cincinnati Airport sto board Delta Airlines Flight 286.
Flight 286 departs Cincinnati at 8:18 a.m.
'1 (EDT) and arrives at Chicago's O' Hare International Airport at 8:15 a.m. (CDT).
Upon deplaning at O' Hare Airport, telephone the United Limousine Service at 969-3865. United Limousine Service will drive you to our office. ~Tell the driver to take you to the Nuclear Regulatory Commission at Building No. 4 799 Roosevelt Road, Glen Ellyn, Illinois.
I anticipate your arrival at our-offices at approximately 9:30 a.m. on September 11, 1984. Our offices are-on the first floor of Building No. 4 at 799 Roosevelt Road, Glen Ellyn Illinois.

I also anticipate that we will need approximately three hours to discuss your l
information; theiefore, I have scheduled a limousine pick up for you at 1:45 p.m.
United Limousine Service will return you to Chicago's O' Hare International l
Airport in time for you to board Delta Airlines' Flight' 245 at 3:15 p.m. (CDT).
Flight 245 is scheduled to arrive in Cincinnati at 5:10 p.m. (EDT).
You should plan to bring with you any documents that will assist us in resolving-We will photocopy the documents and return your copies to you your concerns.

You should also bring with you sufficient cash to defray your airport parking expenses in Cincinnati, your telephone call at O' Hare Airpor,t to United Limousine Service, and for the limousine.
I estimate that your out-of-pocket expenses should not exceed $35 based upon the following estimate:
