Opening Statement: - g ;pyA@pl6g}984 - 10:30 A.M.

' Frank Rolan: This meeting was called as a result of a meeting last Tuesday, .

4/24/84, with Mr. Cosaro. He asked that we form this committee so that we can iron out probisms and see if we can't work closer together without altercations.

Q Leo B: How does QC select the areas to be inspected?

A Frank R: QC inspects tray, then hangers. Pat Foss is giving directions for cable pulling. ,

S Leo B: The best work got done when we had in-process inspection.

Q Jim C: Jim mentioned the fact of in-process also. If welds could be inspected right after the installation, no ICRs would have to be written. Also, to what criteria should they be welding?

, A Irv D: We are working toward complete in-process as soon as backlog of ICRs is cleaned up, about 1,000, and enough inspectors are hired,14 more. They should be welding _only to Procedure 4.3.3.

3 Q Bob C: He feels that the foremen are the first inspectors. They inspect to

' 4.3.3, installation procedure.

A Irv D: Inspectors inspect to 4.8.3, but welders do not use it. Carl Mannecke brought up the fact that what Stu Kleven from S & L stated at ths first meeting was merely informational and not to be used as welding installatton criteria. .Wald to procedure 4.3.3 only.

Q Leo B: Why aren't welds, installations, etc. not OK today if they were when they were first installed?

A Frank R:

The NRC stated: If the Installation Report is not turned in to the office, then you can still be in process and have QC backup, otherwise an ICR aust be geograted on corrections. '

Q Mike B: Why can't welds be inspected in-process?

I. DeWald is working on this by hiring 14 new inspectors. Right now, he doesn't have enough people.

Carl M: QC is also working on training inspectors in welding and configurations so that only one inspector will be needed at an installation. In-process is the best way, of course.

Q Leo B:

Be would like ICR's coordinated all in one area so that the welders don't have to run from one and of the plant-to the other.

A Frank R: All ICRs have been called in.

Irv D:

We have been working overtime on groups of ICRs as Leo brought up cleaning them up. Also, we are doing walkdowns of risers and panels doing complete l

inspections. Wrong bolts were found in some places.

Q Unrk: What is Installation Report is missing?

A Carl M: Be sure old Installation Report is really missing. If two show up, be sure to destroy the one that is not complete.

Q Jake: Do all the red flags in the field mean that an ICR is written?

A Irv D: No. Some of the flags may not have paper because of the Inspector  !

Re-Inspection Program. l Q Jim C: If two inspectors look at the same installation, we will have two pieces of paper. Couldn't the same inspector do the same areas all the , time?

A .We are hiring a training coordinator. We need 22 inspectors to be Irv D:

i A Bob S: 60 to 70% of our inspectors are working on special projects, backlog, t re-inspection, etc. Attrition rate has been terrible lately. This prdblem has hopefully been solved.

Frank R: The craft attrition is the same. We've gone through 2,000 electricians since 1979. -

Q Paul G: Inspectors are coming clear back to the office to look at prints when the foreaan has the prints in the field?

A Irv D: We were told the foreman refused to show them the prints. Hopefully, we will work together in the future. -

Q Paul H: He noticed the problem of the inspectors not showing up when they say they will and not notifying anyone when they can't be there or return when they promised?

A Irv D: This will be brought up and solved as soon as possible.

. Frank 1: That's why we would like to have one certain inspector assigned for each area so that this won't happen.

Carl M: All ICRs that have been called in,1,000, are to be separated into two categories. Welding and all other. problems. S & L is going out to

, overview the welds to see if QC is calling them right. All new ICRs will be evaluated in this way, also.

Pcgo 3 Carl M: The biggest part of inspecting is conduit and cable pulling. They

are pushing to get all t'he cable pan runs inspected s'o they can pull cable. Document packages are eating up time. '

Q Leo B: If we so through the pan one more time, will it finally be done?

We hope we can do it in-process this time.

Q Paul G: During the next few weeks, should we set on WIRs? l A Irv D: No. We will just have to keep up the way we are until enough inspectors are hired for in-process.

Jim C: You promised this by May 1,1984; that is, in-process inspections.

Q Bob S: Are the ICRs valid? If they are, than we have installation problems, and not inspection problems. ,

A Leo B: In the old days, we would weld to the criteria of the inspector for a certain area. ~ When these areas are re-inspected by a different inspector, they are written up. Welding is a judgment call. Irv DeWald rejected 932 of welds in area 426. Different welders maka different welds just as inspectors see the welds differently.

Q Paul H: He wants this job 'to go on for the sake of all the people employed here.

He will do everything he can to make sure his people cooperate with QC.

A Carl M: We will try to iron out all the problems. F. Rolan ordered 100 fillet gauges and 'a training class will be held on the use of these to make sure everyone is using them in the same way. Sample problems will be made up.

Q Paul G: Brought up the fact that the welder used the fillet gauge but the inspector did not know how to use it. He did not know the inspector's name.

Frank R: S & L drawings are not the criteria for the welders to install hangers.

An ECN aust be in their hands before installing anything. You can fabricate, put it together, but do not install it.

j Q Mike B: There seem to be many differences in welds. We seem to use a lot of time

polishing for inspection.

A Irv D: The welds need to be only clean enough for inspection. Don't waste time polishing.

Q Jim C Please have persons who are out inspecting, whether S & L or QC, not get i

all excited if they find hangers and parts lying on the floor, etc., but have them come and check with the area foreman. This will save slot of unnecessary paper.

A Irv D: If you are working in a certain area, you, specifically, will be given the ICRs to correct.
