Subject:

Changes (dated 9/27/86) to the CSR Database In the ASLB hearings for Braidwood on October 1, 1986, you requested additional detailed information regarding the 9/27/86 changes to the CSR database.

In this memo, I will describe the reasons for these changes for those items you specifically identified in your 10/01/86 cross examination of Dr. Kaushal.

Additionally, I am attaching the notes that I generated in my review of the database on September 27, 1986 from which I will be able, if you request, to more fully describe the changes to the remaining items for which specific questions were not previously raised by you in the hearing room.

6 Reasons For 09/27/86 Changes To CSR Database CRP-031 During the review of the database described in P. Steptoe's letter to R. Guild dated September 30, 1986, two additional LKC weld inspection records were identified as applicable to pan CBP-031.

These records were added to the database, with the number of welds

-(and associated inspection points) correctly apportioned between the three LKC inspections.

FJR inspected 2 welds on pan-to-hanger-13H7 welds on 9/1/82 and 4 welds on 21 HV1 on 10/27/81.

IFD inspected 4 welds on 6/10/81 on 15Hl. No inspection record was available for pan welds to hangers 13H21 and 14H7, which accounts for the reduction in the total number of welds by four.

00H-11 1PD'S welding inspection on 7/18/81 was deleted because the Q.C.

inspection record failed to identify which WH-3 hanger was inspected.

In this case, more than one conduit hanger on the drawing was identified as WH-3.

However, the other weld inspection record by WW on 7/18/81 did specifically identify by location the WH-3 hanger reinspected by CSR.

00H-46 In this case, two conduit hangers had identical designations (TS7-3).

By a review of the S&L drawings and the LKC records, I determined that the inspection by LKB applied to the hanger not i

reinspected by CSR.

00H-64 Welds were reworked after the inspection by JBH on 7/6/79 and subsequently reinspected for welding by MAK on 3/13/84. Therefore, the inspection by JBH was no longer applicable.

Welds were reworked after DT's inspection and subsequently reinspected by TML on 9/13/83. DT's inspection was therefore not applicable.

In this case, more than one conduit hanger has the identical 1

designation (WH-10). By a review of the applicable S&L drawings and the LKC inspection records, I determined that the inspections by JBH and RY apply to a conduit hanger not reinspected by CSR.

CPH-160 The record of weld inspections by AD applied to cable-pan-to-cable-pan-hanger welds, which are not included in the CPH reinspection by CSR.

EIN-017 FJR's 6/9/84 configuration inspection was not a full configuration inspection, but only a determination that the junction box was installed. This lack of a configuration inspection for non-safety-seismic junction box supports was described in the BCAP final report.

EIN-112 RY originally performed some welding inspections in 1980 for this equipment. A CECO General Office QA audit performed in September 1983 identified certain deficiencies in this welding documentation.

As a result of this audit, LKC inspector FJR reinspected the equipment welding on 1/18/84 and initiated LKC NCR 1720 which documented welding discrepancies. Since FJR's inspection superceded RY's inspection, RY's inspection was deleted from the database.

However, FJR's inspection was not included in the database because FJR never signed the Weld Inspection Checklist (Form 19) indicating a completed inspection. Also, the NCR 1720 was not closed until June 1986.

EIN-114 This weld inspection applied to a cable support bar, which was not reinspected within the EIN population, as described in P. Steptoe letter to R. Guild dated 9/16/86 for a similar situation. This record was therefore deleted. No other weld records were found that were applicable to this equipment.
