1.

The requirement for covers on junction boxes is excluded.

During construction the junction box covers are frequently removed for cable pulling, cable termination, and by Ceco's testing / maintenance people. Therefore, a L. K. Comstock Q.C. accepted junction box may not necessarily have a cover properly installed during BCAP reinspection.

2.

The requirement of mounting terminal boxes 4'-6" above the floor is excluded.

The requirement is intended for maintenance purposes and is not design significant.

The requirement for drip shields inside containment terminal boxes is excluded.

The environmental qualification testing (Ref: Wyle test report 817657. dated 12/21/83) tditch determined the ability of the subject terminal blocks to perform their safety function under design conditions did not utilize drip shields. Therefore the drip shields are not design significant (

Grouting under front and rear of cabinets as shown on the electrical installation drawing is excluded per TIR 148.

For anchor bolt installations, bolts are specified to be snug tight.

In order to assure this the nuts are installed with a wrench and are not loose. Reinspection to verify that the nuts are not removable by hand will assure that the nuts are not loose.

Internal " point to point" wiring revision work verification is excluded. Commonwealth Edision company's Operational Analysis Department testing and Start-Up testing provide sufficient evidence of correct internal " point to point" wiring revision work.

10.

Bolts and cap screws (not requiring spring nuts) will not be torqued

Segregation code labeling does not affect safe shutdown of the

When separation requirement of minimum 3" metal to metal horizontal and l' vertical between safety and non-safety cable pans, and 5' vertical between redundant ESF division cable pans cannot be met pan covers are required to be installed. No permanent cable pan covers are installed as of June 30, 1984. Therefore, this attribute is not included.

._ Also L. K. Comstock Company B. P. 4.3.5, Sec 6.1 provides for walkdown for determining cover requirements only after all cable pulling is complete and cable pans are cleaned up.

Torquing of Cap Screws on Horizontal Cross members for Cable Pan Hangers (Ref: STD EB-115.rev dated 4.21.83 para 3.7, Comstock Q.A. Procedure 4.3.5 Rev. B, para 3.7.1)

Cap screws with spring nuts installed prior to 2-29-84 will not be torqued to verify the torque values specified in the referenced standard as Ceco QA Audit Report # QA-20-83-59 and QA Surveillance Report #3753 has identified the nonconformance for this attribute. Further, the Surveillance will remain open until L.K. Comstock completes the retorquing program and the Q.A. follow-up is complete. The Q.A.

follow-up will demonstrate that corre.ctive action is complete and action to preclude repetition is in place.

A generic observation had been written to address torquing of cap screws installed prior to 2-29-84 and not covered by the Ceco QA Audit Report #

Bolt spacing Tolerance A tolerance of 1/16" is allowed for the spacing between the centerlines of two. bolts. This tolerance is based on the standard bolt hole diameter which is 1/16" greater than the nominal bolt diameter.

separation requirement between instrtment conduit and power or control conduit in the same safety division (Ref: EB-146 B/B, dated 5/2/84, Section 7.1.13).

This separation requirement is not mandatory by any national code or standard. All class 1E instrument cables are shielded, twisted pair and are installed in their own enclosed raceways. These design features will minimize noise signal interference from power or control cables. Also, S&L has deleted this requirement from the std. EB-146 B/B, dated 8/10/84. Therefore, it is not design significant.

2.

Grounding Requirement for conduits (Ref: EB-146 B/B, Sec. 9.0)

Grounding of conduit is required for personal safety. It is not a nuclear safety related requirement and therefore, it is not a design significant attribute.

Grounding of instrument cable shield is included under cable population.

3.

Conduit Penetration of Fire Barriers (Ref: EB-146 B/B, dated 5/2/84, Sec. 7.1.18).

No fire barriers are installed as of this date. Therefore, this attribute can not be inspected.

4.

Segregation code Labeling at every 15' interval-(Ref: Dwg. 20E-0-3390, Rev. AS, Note 13H).

Conduit segregation code labeling does not affect safe shutdown of the plant even though it is a regulatory requirement. Segregation code marking at conduit terminations, and both sides of floors and walls will be inspected. This will provide reasonable assurance that segregation code labels were installed on the conduits. Therefore, deletion of inspection requirement for 15' interval is not design significant.
