Invalidation of Observations The observations #CSR-I-E-EIN-112-02, CSR-R-E-CND-110-01 and CSR-R-E-CND-XXX-84 were determined to be invalid by the CSR engineers for the reasons. states in Part 3 of the observation forms. The Level II inspectors who wrote these observations had declingd acknowledgment of the invalidation of these observations. Therefore, as required by BCAP-06, Section 4.4.5, a committee consisting of Mr. G.M. Orlov, Mr. L. Shea and Mr. D.J. Patel discussed disposition of these observations and recommend that you or your designee sign these observations as invalid.

Mr. L. Shea, the Level III electrical inspector concurs that the reasons stated in Part 3 of these observations are sufficient to justify invalidation of the observations.

Mr. L. Shea has agreed to sign these observations on-behalf of you upon receiving your approval.

Invalidation of CSR Observations for Unavailable Configuration Inspection Forms - Conduit and Cable Pan Hangers and Conduit Strap Plates Prior to November 1982 L.K. Comstock ahf. E.C. Ernst Co. inspected safety related conduits and cable pan hangers for configuration on a sampling basis.

After November 1982 configuration inspection was performed for all safety related hangers. However, welding inspection was always performed for all safety related hangers. This was documented in population descriptions for both of these populations and was the basis for CSR sample selection.

The CSR inspectors wrote observations for unavailable configuration inspection forms for conduit hangers and cable pan hangers.

The CSR engineers identified these observations to be invalid b0 sed on population descriptions which reflected the contractors quality control program in effect at the time the hangers were installed.

Also, there are seven instances for which doctamentation for conduit strap plate ir)sta11ation was not available. The conduit strap plates are attached to hangers to facilitate conduit installation. 1;s configuration inspection forms for these hangers were not available for reasons stated above the conduit strap plate documentation could not have been available. Therefore, these observations Mre airo determined to be invalid by CSR engineers.

For conduit hangers, 41 of 130 CSR sample hangers had unavailable configuration inspection forms. For cable pan hangers, only 2 of 138 CSR scmple hangers had unavailable inspection forms.

All these cases pertain to work done prior to November 1982. Thus the lack of doctamentation is not inconsistent with the contractors quality assurance practice at the time of the work was done.

l The BCAP Level III inspector (Mr. L. Shea) concurs that these observations identified on the attachment, may be appropriately considered invalid.

However, the BCAP Level II inspector who originally performed the documentation review did not concur. As required by BCAP-06, Section 4.4.5, a committee consisting of Mr. G.M. Orlov, Mr. L. Shea and Mr. D.J. Patel discussed disposition of these observations and recomunends that you or your designee approve these observations as invalid.

Mr. L. Shea has agreed to sign these observations on behalf of you upon your approval.
