SUBJECT: REPORT NOS. 50-327/86-08 AND 50-378/86-08

This refers to a special NRC team inspection conducted during the period

January 21-24, 1986, of activities authorized by NRC Operating License

Nos. DPR-77 and DPR-79 for your Sequoyah facility. This inspection was conducted

by NRC personnel from the Offices of Inspection and Enforcement, Nuclear Reactor

Regulation, and Region II. The inspection was conducted at the TVA Offices in

Knoxville, at the Watts Bar site, and at the Sequoyah site. At the conclusion of

the inspection, the findings were discussed with those members of your staff

identified in the enclosed inspection report. This inspection related only to

the Employee Concern Prcgram as implemented prior to January 24, 1986.

The inspection consisted of a programatic review of the structure and implementa-

tion of the TVA program for determining which employee concerns apply to Sequoyah

Nuclear Plant and which of these concerns are safety-related and/or safety

significant. This review included an assessment of whether TVA had established

and implemented appropriate sc?eening criteria to make these determinations and

to evaluate the generic applivability to Sequoyah of employee concerns at

other TVA nuclear sites. Aread inspected consisted of selective examination of

j procedures and representative records, interviews with personnel, and observa-

tions by the inspectors. While screening procedure weaknesses were noted at

each organizational level, the majority of safety-related/ regulatory issues were

being addressed by the Sequoyah site. 1

Within the scope of the inspection, no violations or deviations were identified. 1

Your attention is invited to the observations made in the inspection report. Our

observations and youtf commitments resulting from this inspection have been

transmitted by a previous letter dated February 13, 1986. Your position with

regard to our observations should be included in revisions to your Employee

Concern Program. These matters will be pursued during future inspections.
