SUBJECT:

10% Visual Wold Inspection When performing the required 1 visual weld inspection of con-tractors welds, the following steps vill be followed.

If, while doing the 10?4 inspection 'and unacceptable indications are found an additional 1575 sample will be taken.

If this sample is acceptabl,e no further inspection is needed.

However, if more errors are discovered a 10096 reinspection of all welds in the area will be required.

If'there are any further questions regarding this matter feel free to contact the site Quality As urance department.

Draidwood Station overview Group - Current Status and Update This letter is being written to inform all involved contractors of the current status of the overinspections being conducted on site..

and o,f the anticipated changes necessary to expand the existing program.

Currently, Pittsburgh Testing Laboratory (PTL) personnel are 1

performing an overinspection of approximately lot of certain quali,,ty control activities at Braidwood.

The expanded program will make an attempt to overinspect 25% of all current contractor activities.

To a c c o mmo. ---date this change, certain ainor revisions to the contractors distribution of quality control caports and related logs may be warranted.

It should be noted that the majority of overinspections to be conducted will be on' accepted contractor Q.C.

activities. with the balance including a portion of in-process reviews (if applicable) and periodic checks of documentation control practicos, welder qualifications. etc.

Any first line Q.C. activities already being covered by PTL 1

(i.e. CEA inspections) will not be re-inspected by the overview group.

In regard to differences in opinion between PTL and the contractors Q.C. disposition, it used for resolution of these items.is intended that a third party be To achieve a certain degree of confidence t both the contractor Q.C. and PTL are reviewing the j

PTL Overview of LKC Welding Mr. Dewald:

i PTL is changing its method of performing LKC overview.

Our present practice is to perform a 10% insrection of 100% of the installations requested.

PTL will begin to inspect 100% of approximately 25% of the requested installations.

In conjunction with this change PTL has developed a new logging system, which will be up-dated as requests are received.

For every one-hundred requested installations PTL will randomly select twenty-five.

The selection process will be in such a manner, that all assemblies on a given request will be selected.

For example:

Request # 1, 10 hangers Request # 6, 2 hangers Request # 2, 10 hangers Request # 7, 20 hangers Request # 3, 5 hangers Request # 8, 10 hangers Request # 4, 10 hangers Request # 9, 10 hangers Request # 5, 20 hangers Request #10, 3 hangers To be overviewed by PIL:

Request #1, 4, 6,'& 10 Requests returned to LKC with no further action necessary by PTL:

2, 3, 5, 7, 8, & 9.

For those installations selected, PTL will perform 100% weld inspection.

Plus, in order to provide better traceability, if a weld is identified as rejectable, a copy or sketch of the appropriate weld detail will be attached to the PTL report as page 2, which will be marked in such a manner as to clearly identify the rejectable weld.

PTL has entered into the new system all LKC requests that have been received at PTL, but yet not inspected.

Those requests that are not selected will be returned to LKC in the near future.

PTL's new system will go into effect for all inspections performed after 11-2-84 If there are any questions plasse contact me.

LKC Overinspection Coverage Please inform LKC that Site Q.A. has directed PTL to increase l

their sample of overinspections f rom 10% to 20% of LKC's visual welding inspections.

This action is a result of a re-analysis of overinspection visual weld rejections from August. 1984 to May, 1985i As a result of this re-analysis a acre accurate trend was provided (see attachment).

The previous trend reported in BRD Letter #15.780 dated 4-26-85 4

reflected accepted and rejected PTL inspection reports.

This to-analysis trended the actual number of welds accepted and rejected.l 1

Results for November 1984 February 1985, below the 90% acceptance rate threshold that was established for the 'andM Overinspection Program.

Future overinspection results will be trended and reported monthly in accordance with PTL's ovarinspection instruction sheet IS-BRD-27-OI Rev.
