m. ~ 0"*'". PHASE I sub-structure 12/75 - 2/77 PTL first arrived at the job site in Dece:her of 1975 with Steve Bank. Project Liaison and J. Troutman. Site Manager. PTL's Quality Assurance manual received fcr:21 approval in February, 1976. In March of 1976. PTL began construction cf the on-site testing lab. With the start of construction. PTL began testing concrete, soils, concrete aggregate, rebar and cadwelds. In June of 1976. PTL instituted full-time en-site QA to perform audits and surveillances of all PTL functions. On October 28. 1976 cur QA manual was revised to reflect more accurately our organizational responsibilities and to better delineate the codes and standards used to accomplish our various quality assurance activities. During this period. PTL staffed 29 personnel en site. A total of 91 audits and 124 surveillances were conducted on 23 procedures and 7 instruction sheets. Also during this period. (October 1. 1976) PIL invited CCRL (Concrete and Cement Reference Laboratory) of the U. S. Government on site to audit and certify our concrete lab and field operations. PMASE II Super Structure 2/77 - 3/78 During this phase. PTL increased its on-site work force to 68. With PTL taking over testing operations at the Byron Site, we changed Site Manager and Soils Supervisor as these two ;erscnnel were transferred to the Byron Site. On June 21. 1977 our QA manual was revised to reorganize its format to coincide with 10CFR-30. Appendix 5. During this period PTL began radiographic slurry, structural, visaal and nagnetic particle inspection. .. "r --f 200 :; fit: :nd 219 t.rveillances on 25 ;receduret a-d 9 instruction sheets. PHASE III Major Equioeent Insta11atio-3'74 - 9/79 During this phase PIL increased its on-site staff to 80. PTL acquired the test and ins;ection contra:t for the *7PS SATSOP cperatiens and. consequently, reorganized the Braidwcod staff to coincide with this additional 51A0010751

F Phase III (continued) activity. PTL replaced its Site Manager as well as its NDE, Lab. Concrete Supervisor at Braidwood. It must be noted that our NOE Supervisor became our Site Manager. We also acquired a new Corporate NDE Level III during this period. During this period PTL began testing C.E.A.s and also began ultrasonic inspections. PTL's on-site QA staff conducted 209 audits and 93 surveillances on 28 procedures and 15 instruction sheets. 4 During September of 1979. Ceco ceased construction at the Braidwood Site. However. PTL kept a skeleton crew to finish various inspections and tests which were began prior to the shutdown. PRASE IV Shutdown 9/79 3/80 4 No activity. McVever. on December 19. 1979 our QA canual was revised to update the procedure listing and to reflect changes in CECO QA philoscphies as well as enhancing the PIL program to more accurately reflect our day to day operation. PRASE V Restart and Regroup 3/80 - 3/81 PTL returned to on-site operations in March of 1980 with a staff of 27. Of these 27.17 vere retained from prior to shutdown. All supervisory and ranage-ment personnel were retained. At this time, all safety-related soils work was completed as well as all slurry testing. Early in 1950, the cooling lake was being filled. All rebar and cadweld tensile testing was referred to LaSalle Station. PTL continued to perform all inspections that were performed prior to shutdown. During this period, the on-site QA staff performed 67 audits and 23 surveillances on 26 procedures and 19 instruction sheets. 4 Wo. -Ju r e_m.. w.....u... ...o During this period. PTL increased its staf f to 35 personnel. On June 1.1982 PTL's QA manual was revised to upgrade the Ccrperste organiza-tional chart and to better deli eate the duties of the on-site CA personnel. In April of 1931. CCRL (Cement and Concrete Reference Lab) was again invited on site to certify our concrete field and lab operations. 1 2-Aoo207sc ~,

PTL also acquired a new Site Manager. c PRASE VII Profect Manager Reorganfration 9/82 - 10/83 During this period. PTL iacreased its sta'f to 55 personnel. PTL began the ANSI-N-45.2.6 enhancement progran in Novecher.1982 and created the position / of Training Coordinator. Later during this period. PTL reorganized the Site / operation to include a Site Manaler Assistant Site Manager. Training Coordinator. and an Administrative Assistant. PTL revised the QA manual in June 1983 to reflect the fact that the reference to ANSI-N-45.2 included all applicable daughter documents as requested by CECO. In July of 1983. CCRL was again itvited on' site to certify our concrete lab.end field operation. PTL's on-site QA perforded 106 audits and 11 surveillances on 27 procedures and ~ 22 instruction sheets. PRASE VrII Reorgsnization 10/83 - 11/94 During this period. PIL increased its staf f to 97 personnel. PTL revised the ANSI-N-45.2.6 certification enhancement program to include comments and suggestions from INP0 Williass/ Wagner. NRC, and CECO QA. t PTL began an over-inspection program on various contractor QC activities. During this period.
