RE-CERTIFICATION OF R. MARTIN - SIX MONTH REVIEW Date: December 7. 1982 Rick has been certified in hanger installation inspection and weld inspection.
His performance in both areas were outstanding in the past, but in July of this year his weld inspections fell into a slump. Pittsburgh Testing Laboratory started rejecting more and more of his inspections. Based on t'te rejection rate of his inspections, the QC Manager made the decision to retract his certification in this area of inspection until further training showed impre vement.
Since July, Rick has trained in weld inspections along with maintaining his work-load assignments.
M. Kast has observed Rick in his training and also re-inspected some of Rick's weld inspections and found that, he is knowledgable in welding and can competently perfona inspections, without unduserestraint.
-7 Rick has since taken the new welding proficiency test and passed with a scora of 92b% based on 40 questions. With this it is respectfully requested that he be re-certified in weld inspection and hanger installation inspection.
