To ensure continued use of this pack:ge, an application for renewal (6 copies) of the existing certificate should be submitted at least 30 days prior to the -

expiration date of the certificate.

Unless an exemption'is provided by 10 CFR Part 170.11, an application fee of

$150.00,'as required by 10 CFR Part 170.31, should be included with the request for-renewal. This notice of expiration should not be construed that such notice will be provided in the future. Also, please let us know if you do not intend to request renewal of the certificate.
