Dear Mr. Haverkamp:

On ' January 23, on the l ast day of your inspection, you requested Cintichem to provide you with some additional information.

Enclosed with this letter are two of the items; the Nuclear Operations Task Assignment memo and the Nuclear Safeguards review of the October 1, 1986 operations letter on required surveill ance tests.

You also asked if a two year period had elopsed between the October 1, 1986 Cintichem identified over-due surveillance test and a previous lapse in surveil l ance perf ormance.

We have reviewed this two year period and have not identified other tests not performed as required.

We have also pulled out of our files and are reviewing the ANSI Standard

" Qual Ity Assurance Program Requirements f or Research Reactors - ANS 15.8".

This standard does give some guidance on the questions discussed during your inspection.

REQUIRED SURVEILL#4CE TEST NOT PERFORMED - LETTER:

OCTOBER 1, 1986 The Committee requested that Operations designate one person to be responsible and accountable for planning, organizing, and controlling the surveillance testing program. The Chief Reactor Operator was designated for this task.

INTERNAL PROCEDURE VIOLATION - LETTER: OCTOBER 22, 1986 The Committee requested that Operations review and approve any maintenance to be done on " critical equipment" before the work is performed. Approval should be obtained from the Chief Reactor Operator or a higher level of management. _ Operations _aoreed to define " critical equipment".

AUDIT OF PACKAGING AND TRANSPORT OF TYPE

'B' QUANTITY RADIDISOTOPES AND THE PACKAGE QUALITY CONTROL PROGRAM - LETTER:

SEPTEMBER 30, 1986 The Committee noted that the document ' Certificate of Competent Authority" should have been labeled " Certificate of Compliance".

Clif f Konnerth will review the need to renew this document and if required will submit the renewal. The Committee discussed the overall function of package licensing and it was established that Cliff Konnerth will be responsible for this task.

ITEM 1 will be implemented as written to meet ALHRA. The Committee agreed that it was not necessary to take action on ITEM 2.

AUDIT OF THE M#MGEMENT OF THE HEASUREMENT 00ALITY ASSUR#4CE PROGRN1 - LETTER: OCTOBER 10, 1986 The Committee approved the document as written. The i:CC will begin formal review of the HQA annually.

AUDIT OF SNM CRITICALITY CD1 TROL - LETTER:

OCTOBER 11, 1986 The barrel incident was reviewed by the Committee and agreed that since the barrels had been assayed at each being less than 15gms, they were exempt from fissile Class 1 and did not require criticality control.

The subject of the 19 month delay was addressed in previous minutes and we have been in compliance since July 1986.

The Commi ttee agreed that the weekly alarm check, mentioned in this document, is in fact checked monthly and a monthly procedure is being implemented.

OCTOBER 13, 1986 The Committee recommended that Reactor Operations should increase eHorts to keep more timely records.

11. At44UAL AUDIT REQUIRED 8Y LICENSE R LETTER: OCTOBER 13, 1986 The Ccanittee requested that Operations continue to investigate the cause of "all rod drops.'

The NTD Si project was discussed and the Cceraittee agreed that a full Safety and Hazards analysis complete with a maximum credible accident identification must be sutenitted by Operations and reviewed by the NSC prior to facility start-up. This report should also include an analysis of routine and emergency operations.

All other deficiencies have been addressed under separate headings of this minutes.

TEMPERATURE MEASUREMENT IN NORTH FACE SILICON FACILITY - LETTER:

OCTOBER 21, IMS l'he NSC revicued this document and accepted it as written. A suggestion was made to Operations to run two cans simultaneously.

One can would contain a 4' ingot and the second a 3' ingot.

REACTOR OPERATIONS TASK ASSIGNMENTS This memo is a lIsting of task assignments for the Reactor Supervisor, Chief Feactor Operator, Assistant Chief Reactor Operator and Nucl ear Project Engineer positions.

This listing of specific Job duties is an ef fort to more clearly dif ferentiate Job responsibilities between the above f our positions.

This was necessitated by the presor.t vagueness and overlap of the present job descriptions.

A general description of job responsibilities follcws:

Reactor Supervisor - Overall responsib il i ty and accountab il i ty for reactor operations.

Must insuro that duties of those reporting to him are completed in a satisfactory and timely manner, involved in the non-routine requirements of reactor operations, delegating to the Chief Reactor Operator the daily, routine tasks.

Chief Reactor Operator - To supervise the personnel involved in the operation and maintenance of the reactor.

General direction under the reactor supervisor.

He or the personnel reporting to him handle alI routine operational tasks.

Assistant Chief Reactor Operator - Assist the Chief Reactor Opeartor wIth his

duties, in tho absence of the CRO, the ACR0 will assume the duties assigned to the CRO.

Nucl ear Project Engineer - Assigned project tasks rel ated to reactor or f acil ity operation.

Generally these tasks require technical expertise and involve ongoing operational problems, new projects, or training.

The above descriptions give a brief listing of position respon si b il i ties.

L isted below are Job categories broken down into sub-sections with the appropriate position responsibilities assigned.

The acronyms RxS, G0, ACRO, and NPE will be used to represent the Reactor Supervisor, Chief Reacter Operator, Assistant Chief Reactor Operator, and Nuclear Project Engineer.

Provides schedule and lecture assignments.

Insures that training stays on schedule and that del egated lecture assignments are adequately perf ormed.

Writes, deseminates, and corrects byannual written Reactor Operator requalification exam.

(This can be delegated to another SRO.)

CR0 -

Conducts training exercises assigned by RxS.

Coordinates manipulation performance and manipulation review program.

Insures all l icensed operators perform required manipul ations, etc.

Reactor Operator licensing renewal paperwork and physicals.

NPE -

Conducts training exercises as requested by RxS.

Fuel Management RxS -

Overall f uel management.

Coordinates and schedules new and spent f uel shipments.

Maintain appropriate sel f protection accountabil ity f il e.
