Gentlemen:

This refers to the special inspection conducted by Mr. T. N. Tambling of this

office on October 8,1986, of activities at the University of Illinois Nuclear

Reactor Laboratory authorized by NRC Operating License No. R-115 and to the

discussion of our findings with you and others of your staff at the conclusion

of the inspection and during the Enforcement Conference held in the NRC

Region III office on October 28, 1986.

The enclosed copy of our inspection report identifies areas examined during

the inspection. Within these areas, the inspection consisted of a selective

examination of procedures and representative records, observations, and

interviews with personnel.

During this inspection, certain of your activities appeared to be in violation

with NRC requirements, as specified in the enclosed Notice of Violation. The

root cause of this violation is your apparent failure to recognize that

continued operation of the reactor with a wiring error that could circumvent

the Standard Control Rod Position interlock violated your license and NRC

regulations. In this case, continued reactor operations with the known wiring

error constituted a change to the description of the interlock in your Safety

Analysis Report and license and therefore required prior NRC approval. Such a

violation could be classified as Severity Level III. However, because of the

low safety significance associated with the wiring error and the fact that

the interlock would have performed its intended function in all normal

operations, the violation has been classified as Severity Level IV. In your

written response to this violation, you should address those actions taken or

planned to preclude this failure to recognize the operability requirements of

your license and need to request formal NRC approval prior to deviating from

these requirements.

In accordance with the Commission's regulations, a copy of this letter and

the enclosed inspection report will be placed in the NRC's Public Document

Room.

The responses directed by this letter (and the accompanying Notice) are not

subject to the clearance procedures of the Office of Management and Budget

as required by the Paperwork Reduction Act of 1980, PL 96-511.
