Dear Dr. Grace:

1 0FFICE OF INSPECTION AND ENFORCEMENT BULLETIN 85 MOTOR-OPERATED VALVE (MOV) COMMON MODE FAILURES DURING PLANT TRANSIENT 3 DUE TO IMPROPEk SWITCH SETTINGS - SEQUOYAH NUCLEAR PLANT (SQN)

This letter is provided in accordance with a telephone conversation between G. B. Kirk and M. A. Purcell of TVA and Bob Carroll and George Sneveley of NRC on October 16, 1986. Enclosure 1 contains the information discussed in the above telephone conversation. Enclosure 2 identifies the comitment made in the telephone conversation and detailed in enclosure 1.

If you have any questions, please get in touch with M. R. Harding at 615/870-6549.

To the best of my knowledge, I declare the statements contained herein are complete and true.

Very truly yours, TENNESSEE VALLEY AUTHORITY R. Gridley, D rector Nuclear Safe and Licensing Enclosure cc (Enclosure):

ENCLOSURE 1 In my letter to you dated September 11, 1986, TVA revised the completion date for items 1 and 4 (shown below) of enclosure 2 to R. H. Shell's letter to you dated May 12, 1986. This delay was based primarily on difficulties we have had obtaining all necessary valve thrust information from valve vendors.

Items 1 and 4 as taken from our May 12, 1986 response are identified below:

1.

Determination / verification of correct switch settings for 21 Movs on each unit in attachment 1 by August 31, 1986.

4.

Stroke test the 21 MOVs on each unit in attachment 1 under nondifferential pressure conditions by August 31, 1986, for verification of correct switch settings as determined for item (b).

As discussed in the October 16, 1986 telephone conversation, one vendor (crane /Alloyco-Walworth) has failed to perform on a contract to provide maximum alloweble thrust data for valves 1, 2-PCV-1-15, -16, -17, -18.

All eight of these valves (four on each unit 1 and 2) are of a common four-inch 600-pound valve design. As a result, we have reviewed the data available for the Walworth valves and established by engineering evaluation / judgement that the actual thrust being applied to the valves in the plant is acceptable (i.e., would not result in damage to the valve). Based on this evaluation we conclude that sith the identification of one additional open item (to be tracked as a separate commitment), items 1 and 4 above are complete.

The open item referenced above results from the fact that although items 1 and 4 are completed as described, one of the valves (1-PCV-63-40) remains under investigation. This particular valve appears to require larger forces to operate than its sister valve. This investigation is expected to be completed by November 30, 1986, and unless otherwise notified, the remaining commitments contained in our May 12, 1986 response to the bulletin will be completed as previously indicated.

ENCLOSURE 2 LIST OF COMMITMENTS 1.

Complete the investigation of thrust requirements for 1-PCV-63-40 by November 30, 1986.
