Dear Sir or Madam,

Please consider this letter as a formal request for all supporting documents used in generating the above-identified reports. These reports concern the fatal ac-cident at the Surry, Virginia Nuclear Power Plant which occurred December 9, 1986.

While I have the reports in question, I am requesting all reports, test results, memoranda, letters and other doc-uments which resulted from the NRC's investigation of the accident at Surry and which were used, in whole or in part, in compiling the above-referenced reports. My request in-cludes, but is not limited to, the following:

1. All internal memoranda, letters and reports exchanged between NRC personnel

2. All NRC correspondence directed to "outside parties",

including Virginia Power, Stone and Webster Engineering Corpora tion, the State of Virginia or any agency or individual of the State of Virginia, outside consultants or other experts, and any other in-individual, company, agency, or other entity contacted in regards to the Surry._ incident.

3. All letters, reports, test results, and other documents received by the NRC from the "outside parties" identified in the previous request.

4. All other documents in the possession of the NRC pertaining to the Surry incident.
