ORGANIZATION The Quality Assurance Program (herein called the program) shall be administered by the Radiation Safety Officer.

Handling, storage, shipping, inspection, testing, operation and record keeping shall be the responsibility of the radiographers who have completed training in the use of the Cobalt 60 Source and have been authorized by NRC License.

The final responsibility for the program rests with Atlantic Research Corporation. The line of responsibility is indicated by the organizational chart of the licensed activity (Figure 1).

II.

QUALITY ASSURANCE PROGRAM This program refers only to the preparation for shipment, shipment, and receipt of shipments of licensed material.

Design, fabrication, and modification shall not be conducted under this program.

Shipment of licensed material to the vendor shall be conducted according to the following guidelines:

NOTE:

The procedures listed herein shall be rigidly adhered to.

The Radiographic Operators shall establish a Log, record the date, the dosimeter readings as required, and verify packaging complete and correct.

The licensed material shall be shipped in the approved Technical Operations, Model 676 or other shipping container approved by the Commission and provided by the vendor.

The shipping container shall have the following information l

clearly visible and durably attached.

1. Source material, quantity, and calibration date.
2. DOT Transportation Group label in accordance with Department of Transportation Regulations.

3. Label stating " CAUTION - RADI0 ACTIVE MATERIAL" in accordance with NRC Regulations.

4. Name and address of origin of shipment.
5. Name and address to whom shipment is made.

The shipping container shall be anchored in such a manner as to prevent bouncing, rolling or other movement during transport.

The licensed material shall be locked into the shielded position and the closure plug shall be installed and lock-wired into position.

e. ' A physical survey shall be made of the shipping container to ensure that the reading at the container surface does not exceed 200 mR and that the reading at three feet from the container surface does not exceed 10 mR. Containers having readings exceeding these amounts may not be shipped under this program.

If any container is received with readings exceeding those above, action shall be taken according to NRC Regulations as defined in Part 20.205.

III.

DOCUMENT CONTROL This Quality Assurance Program shall be incorporated into the existing Document Control System at Atlantic Research Corporation. This assures that all changes are reviewed for accuracy and approved prior to release and distribution for use. All changes must be approved by the Radiation Safety Officer and the Director of Quality Assurance.

RECORDS Written records of all activities relating to this program shall l

be maintained and made available for inspection upon request by the Commission for a period of 5 years. This record shall include as a minimum:

shipment and receipt data of by-product material, date, survey results, and changes in this program.

AUDITS The Quality Assurance Program shall be audited at least annually by the Corporate Safety Engineer to assure compliance with provisions of the program.

The Audit shall be performed using the pre-established check list as required in the By-Product Material License No. 45-02808-04, Section 2310.06(G).

The Safety Engineer shall have no responsibility for the implementation or administration of the Quality Assurance Program.

VI.

CORRECTIVE ACTION Any items of non-compliance shall be reported by the Safety Engineer to the Director of Quality Assurance and to the Radiation Safety Officer.

Corrective action must be taken within two weeks of notification of non-compliance.
