OPERATIONAL SAFETY ASSESSMENT As a result of the accident involving the release of uranium hexafluoride from Kerr-McGee Sequoyah Fuels Facility in Gore, Oklahoma, on January 4,1986, the NRC has decided to conduct an operational safety assessment at each major fuel cycle facility. The assessment of your Lynchburg facility will be conducted by a team of six or more inspectors. The team will be composed of representatives from Region II, IE HQ, NMSS, EPA and OSHA. This assessment has been scheduled for the week of November 3, 1986. The team leader, Mr. John B. Kahle, will contact your office at a later date to arrange a time for the entrance briefing.

During the assessment, the team will examine existing operational conditions at your facility to identify potential safety hazards that, when combined with site' operations, could impact public health and safety. The team will also examine whether there are systems and procedure's in place to identify the use and handling of hazardous materials which could result in radiological safety consequences, and will determine whether these are adequately implemented. The areas to be covered include emergency contingency planning, procedures control, training, audits, management review, radiation safety, chemical process safety, fire protection, nuclear criticality safety, maintenance and surveillance as they relate to the various operations conducted at your plant.

As a result of the assessment, the NRC will determine if additional measures are necessary to address safety problems identified during the review.

The assessment is not an inspection; it is intended to provide feedback to both the NRC and the licensee.

However, if a violation of an NRC requirement is identified during the assessment, it will be handled in accordance with our Enforcement Policy., Violations of EPA and OSHA requirements will be handled through their respect-ive agencies.

Records of reviews and audits conducted by your personnel, maintenance, training, and contingency plan drills should be made available for review by the assessment team while at your facility.

In order to facilitate the assessment at your facility, please have available for perusal by members of the team copies of your existing procedures which cover employee training, equipment maintenance, nonradioactive chemical control, fire protection, operational safety and contingency planning. Also, we request that a conference room or similar space be made available for the team to meet as a group to discuss and summarize findings.
