NOTICE OF VIOLATION Toledo Edison Company Docket No. 50-346 As a result of the inspection conducted on June 16 through July 31, 1986, and in accordance with the " General Policy and Procedures for NRC Enforcement Actions," 10 CFR Part 2, Appendix C, (1985), the following violations were identified:

1. Technical Specification 6.8.1 requires that written procedures shall be established, implemented and maintained. 10 CFR 50, Appendix B Criterion V states in part, " Activities affecting quality shall be prescribed by ... and shall be accomplished in accordance with these instructions, procedures, and drawings". Section 5.1.1 of the licensee's Neclear Quality Assurance Manual (NQAM) states, " Division directors performing activities covered by this QA Manual shall ensure these activities are carried out in accordance with approved procedures".

Toledo Edison Administrative Pro;edure AD 1827.06, " Tornado", requires that the Load Dispatcher notify the station that a tornado watch or warning exists for the vicinity of the station. ,

Contrary to the above, a tornado warning was issued by the Weather Service for the vicinity of the station at 9:11 P.M. on June 10, 1986, and the load dispatcher did not notify the station.

This is a Severity Level IV Violation (Supplement 1).

2. Technical Specification 6.2.3 requires that no person performing safety related work should be permitted to work more than 16 hours1.851852e-4 days <br />0.00444 hours <br />2.645503e-5 weeks <br />6.088e-6 months <br /> in any 24 hour2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> period or more than 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> in any 48 hour5.555556e-4 days <br />0.0133 hours <br />7.936508e-5 weeks <br />1.8264e-5 months <br /> period, or more than 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> in any seven day period without authorization and the basis for granting the deviation by the superintendent.

Centrary to the above, between June 7 and July 11, 1986, two I & C personnel worked more than 16 hours1.851852e-4 days <br />0.00444 hours <br />2.645503e-5 weeks <br />6.088e-6 months <br /> in a 24 hour2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> period,15 individuals worked more than 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> in a 48 hour5.555556e-4 days <br />0.0133 hours <br />7.936508e-5 weeks <br />1.8264e-5 months <br /> period, and 22 individuals worked more than 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> in a seven day period.

This is a Severity Level IV Violation (Supplement 1).

Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or l

explanation in reply, including for each violation: (1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further violations; and (3) the date when full compliance will be achieved.

Consideration may be given to extending your response time for good cause shown. ,
