Recently, a num'oer of technical and regul'atory issuos have arisen concerning the use of Limitorque motor operators. The staff is concerned with the reliability of these valve' operators to perform their intended function;in a safety related

application in a nuclear facility.' Within the past year, Limitorque motor cperators have been the subject of three'IE Information Notices and an NRC inspection (Report No. 99900100/86-01)'. With regard to the inspection, the NRC letter dated November 7,1986 requested a response to the Notices of Violation and Nonconfermance by December 6, 1986. As of this date, we have yet to receive Limitorque's reply. Accor(ingly, the staff is requesting a meeting with Lipitorque management at our Bethesda, Maryland effices to discuss this item along with the following issues:
1.
IE Information Mtice 87-08, " Degraded Motor Leads In Limitorque DC Motor Operaters."
2.
Warped torque switch rotors sent to Sacramento Muricipal Utility District for Rancho Seco in January 1987.
3.
Torque switch imbalance reported by Toledo Edison for the Davis Besse nuclear facility in February 1987.
4 Criterie supplied in August 1986 for exceeding thrust limits on operators supplied to Arkansas Power and Licht for the Arkensas Nuclear One facility.
5.
Cracked grease seals on Reliance rotors supplied to Southern Cal Unrnia Edison for the San Onofre nuclear facility and returned to Linitorque in February 1987.
6.
Qualification of Marathon 1600 type terminal boards installed in some Limitorque Motor Actuators.
7.
Limitorque's notor operated valve actaator testina progran.

Recent motor burnouts at various utilities (e.g., Brunswick).
9.
Retrofit kits for preventing hydraulic lock caused by leakage of EP-0 type grease.
- 10. Shaft / keyway deficiencies found in type SMB-3 valve operators equipped with 100 ft.-lb. motors at the Watts Bar nuclear facility.
- 11. Errors in calculating torque switch settings for type SMB-000-3 and ST valve operators reported by Carolina Power & Light Company for the Brunswick nuclear facility in March 1987.
This matter has been discussed by my staff with Patrick McQuillan of Limitorque and it was agreed that the meeting date would be best set after Limitorque had an opportunity to review the above list of issues.
