NONE There were no unit shutdowns or significant power reductions during the month. The unit has operated at a nominal 90% power limit administratively imposed to minimize steam generator tube degradation.

1 of 1 MONTHLY OPERATING ACTIVITIES - OCTOBER, 1986, HIGHLIGHTS:

The reporting period began with the Unit at 90% rated thermal power.

The Safety Parameter Display Systen (SPDS) was unavailable most of the reporting period due to hardware problems.

The reporting period ended with the Unit at 90% rated thermal power.

The gross electrical generation for October was 668,550 MWH.

SUMMARY

10-4-86 At 0857, all Eberline Radiation Monitoring System (RMS) channels were inoperable due to both Control Room

-Control Terminals being inoperable, causing the Unit to enter action statements for Technical Specifications 3.3.3.10 a.nd 3.4.6.1.

All Eberline RMS channels were declared operable at 0947 the same day.

10-14-86 At 0737, the Control Room received an " Urgent Failure" alarm on the Rod the Control System due to 3 blown fuses and a failed amplifier board in the logic cabinet.

10-22-86 At 1238, a power reduction was initiated as required by Technical Specification 3.0.3 due to an apparent environmental qualification problem with Auxiliary Feedwater System Supply Valves (FMO 212 and 232).

The concern was resolved and the power decrease terminated at 87%.

Reactor power was restored to 90% at 1434 the same day.

50 - 315 UNIT NAME D. C. Cook - Unit No. 1 DATE 11-6-86 COMPLETED BY B. A. Svensson TELEPHONE (616) 465-5901 PAGE 1 of 1 MAJOR SAFETY-RELATED MAINTENANCE OCTOBER, 1986 I&C-1 Received urgent failure alarm on rod drive control system.

Found urgent failure alarm indicated on power cabinets 1BD and 2BD and found 3 blown fuse indicators. Replaced blown fuse in-dicators and reset urgent failure alarm. When an attempt was made to move the rods, the urgent failure alarm was again received.

The problem was traced to a defective AC amplifier board at slot A812 in the rod control logic cabinet. The defective AC amplifier board and urgent failure alarm were reset and proper operation of the rod control system was verified.
