Pursuant to the telephone conversation on August 1,1986 between Mr. James T. Conway of this office and Mr. William Miluszusky of Limitorque, this is to confirm the inspection of your facility in Lynchburg, Virginia, on September 8-12, 1986. The inspection team will consist of five inspectors. .
In addition to a review of the overall implementation of your QA program, the inspection will include the following items pertaining to Limitorque operators as identified in Part 21 reports to the NRC:
- 1) Defective motor insulation on H. K. Parker / Peerless motors (Portland General Electric in May 1986).
- 2) Dama ed internal wiring in Type SMB-000 operators (Georgia Power in March 1986 .
- 3) Weight discrepancies in Type SMB-00 operators (Consumers Power in July 1986).
- 4) Cracking of limit switch rotors (Sacramento Municipal Utility District in June 1986).
In addition, a review of the controls imposed upon vendors of qualified equip-ment / parts will be undertaken to evaluate how limitorque ensures that equipment /
parts supplied for environmentally qualified operators have successfully passed qualification testing.
