Dear Mr. Keppler:
This letter is in response to the inspection conducted by Messrs.
M. Jordan, R. Kopriva, J. Mueller and D. Butler of activities at LaSalle County Station. Reference (a) indicated that certain activities appeared to be in noncompliance with NRC requirements. The Commonwealth Edison Company response to the item in the Notice of Violation (Reference (b)) is provided in the Attachment. An extension until February 27, 1987 was granted in a telecon between C. M. Allen and M. Ring of your staff.

VIOLATION - IR 50-374/86040-03 Technical Specification 3.3.2.b requires that the isolation actuation instrumentation channels shown in table 3.3.2-1 be operable or the action taken required by the Limiting Condition for Operation (LCO) which is, "With the number of OPERABLE channels less than required by the minimum OPERABLE channels per trip system requirements for one trip system, place the operable channel (s) and/or trip system in the tripped condition
- within one hour." A foot note at the bottom of the page states, "*An inoperable 4
channel need not be placed in the tripped condition where this would cause the trip function to occur.
In these cases, the inoperable, channel shall be restored to OPERABLE status within 2 hours2.314815e-5 days <br />5.555556e-4 hours <br />3.306878e-6 weeks <br />7.61e-7 months <br /> or the ACTION required by table 3.3.2-1 for that trip functions shall be taken."
Table 3.3.2-1 lists " Action 25" for the Reactor Vessel (Residual Heat Removal (RHR) Cut-In Permissive Pressure-High).
" Action 25" states,
" Lock the affected system isolation valves closed within 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br /> and declare the affected system inoperable".
Contrary to the above, on October 16, 1986, at 12:35 a.m., the Reactor Vessel (RHR Cut-In Permissive Pressure-High) did not have the minimum operable channels per trip system until 10:50 p.m. on October 16, l
1986, and the affected system isolation valves'were not locked closed within the allowed LCO time limit, i
CORRECTIVE ACTION TAKEN AND RESULTS ACHIEVED Upon determination that the pressure switch was not OPERABLE due to the closed isolation valve, the Instrument Maintenance Technicians proceeded 1
to return the instrument to OPERABLE status. This action was completed within the 2 hcurs allowed by Technical Specification 3.3.2.b.
CORRECTIVE ACTION TAKEN TO AVOID FURTHER VIOLATION The LCO requirement (locking shut the isolation valves) was not complied with because personnel were unaware that the switch was not OPERABLE. After the problem was discovered, corrective action was taken in accordance with the Technical Specifications to return the switch to OPENABLE before it became mandatory to lock the valves shut.
The problems which led to this condition, failure of second verification and monitoring of plant status, are separate violations (373/86040-01(DRP) and 373/86040-02(DRP)). Details of the corrective actions for these areas will be supplied as part of the responses to those violations.
