The allegations are related to the ongoing Independent Design Verification Program which is the subject, in part, of Commission Order CLI 81-30. The Office of Nuclear Reactor Regulation requested the' Office of Investigations and the Office of Inspection and Enforcement to review the letter for any action which they deemed appropriate. We have been advised by IE and OI that no investigation is warranted because they are satisfied that the concern identified in the letter will be covered by s Phase II of the Independent Design Verification Program. I am currently involved in the Diablo . Canyon Review effort now in progress., I nust say that this is the . worst nuclear power plant that I' have ever worked on.

and it is not SAFE, I was hoping with the introduction of Bechtel that a complete re-analysis of Unit 1 would be done. Alas thi's is not to be. The pGLE management / engineers only correct the mistakes that are found accidentially or are forced down their throats. If the error is an generic one, they will not review everything . but just what caused the error and they will only correct that one mistake. The latent review is for hosgri only but the real danger.is that:
- 1) The thermal / deadweight analysis does not match the. seimic or current field support configuration.
Systems are failing when corrected.
. 2) The so-called 'As-Built" pipe support drawings ~
are not as-built ~but instead are the accumulation of all the changes to the support drawing over the past 13 years. The support have not been walked down in the field to show thereilatest configuration or orientation 'to the pipe. Also interferences with structure are not show on the drawin5s.
- I only ask that you have R. L. Cloud & Assceiates review the thermal / Deadweight analysis m's well as the .Hos5ri they are now doing. This will confirm what I have said. If action is not taken, I may be force to seek a publi'c hearing on this. 'I am pro-nuclear but invalid analysis andincompetence must not be igored. This palnt in my opinion is bad from the analysis to the supports.

= f-REQUEST FOR ADDITIONAL INFORMATION QUALITY ASSURANCE PROGRAM BECHTEL POWER CORPORATION DIABLO CANYON PROJECT 260 QUALITY ASSURANCE BRANCH 260.1 The June 18, 1982 letter from PG&E states that PG&E and Bechtel have formed an integrated project organization.- However, the enclosure to the letter is titled " Quality Assurance Program - Bechtel Power Corporation - Diablo Canyon Project." Clarify.
260.2 To provide a better perspective of the integrated project organization, indicate by a B (for Bechtel Power Corporation) or a P (for PG&E) on figures 15-17 which organization is responsible to fill each block. 260.3 Discuss how PG&E (as the applicant) assures the quality of the work per-formed by the Project Team and/or by Bechtel (as a principal contractor). For example, do PG&E personnel, apart from the project team organization, function in accordance with the Quality Assurance Program described in section 17.2 of the Diablo Canyon FSAR? 260.4 Differentiate between the responsibilities of the Project Quality Assurance Engineer (with the"QRE's Field Home Office" block below), the Project Quality Engineer, and the APE Quality as shown on Figure 15.

260.5 What individual (by position title) is responsible for the quality of

260.6 The fourth paragraph on page 1 of the Bechtel QA Program indicates that-( procurement may be by either PG&E or Bechtel. Verify that the company l which places the purchase c.rder will be responsible for supplier evalu-l' ation, supplier control, supalier audits, and source inspection, or indicate how these functions will be divided. If PG&E does the procure-ment, will the PG&E procurement controls be in accordance with section 17.2 of the Diablo Canyon FSAR or will they be in accordance with the enclosure to the June 18, 1982 letter? i 260.7 Discuss the involvement of PG&E's plant staff in the preoperational l testing phase activities to assure familiarity with the plant during l early plant operation. l l 260.8 Attachment 8 to the program description states: " Quality Control performs surveillance inspection over preoperational and startup testing." It is not apparent that this organization (as shown in Figure 17) has personnel with the education / training / experience to perform meaningful surveillance inspection. Discuss. Also discuss the involvement of PG&E QC personnel j in preoperational and startup testing to assure familiarity with the l plant during early plant operations.

BOARD NOTIFICATION 82-72 I have determined that.,the attached information concerning the results of a Region IV investigation at Teledyne should be sent to the appropriate Board and parties in accordance with the procedures of Office Letter No.19. The investigation was stimulated by an allegation relating to the Fitzpatrick and Yankee Electric facilities. The results reflect an audit of the Teledyne internal QA program. Since the investigation may be viewed as a cross-cut of the Teledyne QA program, its results reflect the general level of conformance to QA requirements at Teledyne. It is therefore applicable to all plants for which Teledyne is doing work. These plants include Diablo Canyon, Ferme 2, Summer, Limerick, and the operating reactors listed on the first page of the inspection report. DIA8LO CANYON Unit No.1 - Design Verification Program A license for Fuel Loading and Low Power Testing was issued to Pacific Gas and Electr;1c (PG&E) on September 22, 1981. An error i'n Seismic Design of Equipment Installation and Piping in Containment was detected by PGAE and reported to NRC late September 1981. Serious weaknesses identified in the fall 1981 in PG&E Design and Quality Assurance Progran.s raised doubt on the adequacy of the plant to withstand . earthquake motion. On November 19, 1981, NRC issued an order suspending the license and issued requirements for a Design Verification Program to be completed prior to restoration of the license for fuel loading and low power testing. (Referred to as Phase I of the Diablo Canyon Design Verification Program.) On November 19, 1981, the NRC informed PG&E of additional Design Verification Program requirements that must be met prior to the issuance of a full power license for Diablo Canyon, Unit No. 1. The activities related to thet part of the program dealing with the full power license (FPL) are referred to as Phase II. (A Hearing on the FPL was completed in January 1982. In June 1982, a motion was made to reopen the Hearing due to QA problems.) Between November 1981 and March 1982 PG&E reassessed effort, defined scope

On March 19,1982, NRC approved Teledyne Engineering Service Olaltham, MA) as independent contractor to conduct Independent Design Verification Program. (Other participants in the Independent Verification Program are R. L. Cloud, Associates, and R. F. PART 21 REPORT ON PIPESD COMPUTER, PROGRAM The enclosed Part 21 report from Control Data Corporation (CDC) identified an error in the computer program PIPESD used for piping analysis. This computer program has been used to perform piping analyses at Diablo Canyon. The enclosed notification states, "Results from time history load cases and response spectrum cases are unaffected." Therefore, the error should not affect the current seismic analysis work at Diablo Canyon. We have requested that Region IV review the extent of the error during their current inspection at CDC to verify that seismic analysis results will not be affected. Based on the enclosed notification, the error requires several conditions to be present and should apply to a limited number of analyses. However since&the computer program was used at Diablo, there is a' potential that some non dynamic (thermal, deadweight, etc.) analyses could be affected by the error. y 4 Robert L. This letter is provided to report a potential error condition in a computer software program which Control Data Corporation makes available to users of its CYBERNET computer system. Control Data Corporation is unable to evaluate whether this error condition could create a " defect" as defined by 10CFR21, and, therefore, has notified its customers by letter dated July 7, 1982, in order that they may cause the error condition to be evaluated. g The error condition exists in the computer program PIPESD ( tite
" program") which can be used by Control Data customers to pet-form pipe stress analysis. The program operates on Control Data's CYBERNET computer systems, under control of the SCOPE 3.4 and NOS operating systems at data centers located in: Rockville, Maryland; Houston, Texas; Sunnyvale, California; Waltham, Massachusetts.
The program is the proprietary product of URS/ John A.' Blume & Associates, Engineers, 130 Jessie Street, San Francisco, California 94105. A list of customers who were notified is enclosed together with a copy of Control Data's notification letter which has been sent to each of the customers. This list includes both customers who may have and may not have used PIPESD in nuclear work. When modeling an elastic spring support, the PIPESD user must specify several parameters including support grid point number,
- support type, support spring stiffness, and choice of " global" or " inclined" support direction. In the.ev._ent that
- 1) One or more " global" and one or more " inclined" supports, or
- 2) Two or more " inclined" supports are specified on the same support grid point number, PIPESD may incorrectly calculate the support reactions. Results from time history load cases and response spectrum cases are unaffected.
URS/ John A. Blume and Associates has researched the extent of ~ 3 this problem through earlier program versions. It appears to i exist as early as Version 4.0 of PIPESD (circa 1975) and i probably as early as Version 3.0 (circa 1974). URS/Blumehasidentifiedamethodofa~voidingtheproblemuntk a correction can be released. A description of this method is. ~ enclosed as Attachment A. In addition, Control Data may be able to assist you in determining whether your runs of PIPESD have been affected by this condition. A revised version of PIPEhD is being prepared by URS/Blume to , correct the problem which is herein reported. This revised version will be made available to Control Data's CYBERNET users as soon as possible after it is received from URS/Blume.
