ATTACHMENT 1 NOTICE OF VIOLATION 86-46-01 10CFR50, Appendix B, Criterion V requires that: " Activities affecting quality shall be prescribed by documented instructions--and shall be acceplished in accordance with these instructions -."
Fi eld Construction Procedure No. FCP-431, Section 5.2.10 states that: " Cut edges, sharp corners and holes shall be deburred to provide smooth surfaces which will not danage cables."
Contrary to the above, on Decenber 10, 1986 Cable No. 2SISA0H301 in the low head safety injection pump No. 2 SIS *21A housing was identified by the NRC in-spector that had been danaged at the splice junction by the sharp edges of the f abricated stainless steel motor housing base plate.
This is a Severity Level IV violation.
Response
Inspection of installation of Raychem Heat Shrinkable Tubing resulted in the identification of danage to ' cable splices located in the low head injection punp (No. 2 SIS *P21A) motor lead housing.
The 3-Phase SKV type HTK cable spliced to the punp motor leads showed signs of danage from the sharp edge of the housing back plate.
The cable size and stiffness of the cable places the cable against the sharp metal edge when the motor lead housing cover is replaced.
The danage to these cable splices was addressed on N&D-38725, which has been dispositioned to increase the size of this motor tennination box, to replace the danaged splice kit, and to place edge dressing material on sharp edges.
Corrective action will be conpleted by February 23, 1987.
To detennine if this condition exists in other cases, Site Quality Control is currently reviewing all similar field installations.
This review is expected to be conpleted by March 16, 1987.
F o r further installations, N&D 38873 has been dispositioned and resulted in i
revising I.P.-8.5.2 to include criteria for all remaining work.
