0 ATTAO9ENT Ouestion 1 Has the training conducted in response to violations 373/86015-01a and 373/86015-01B been incorporated into the permanent operator training Program? If not, when will this be accomplished?

Response

f The importance of clear communications and procedural adherance is already covered in lesson plans used for training initial license operators and auxiliary operators. A review of this material has determined that appropriate em;;hasis already exists in the permanent training program.

Operator training programs are being revised to include additional emphasis on individual signature authority responsibility. The additional training will bs based on requirements outlined in the new station procedure, LAP-100-28. These program revisions will be completed by September 15, 1986.

Question 2 Was the summary of licensed operator responsibilities prepared by the NSO incorporated into the permanent operator training program? If not, how will this summary be made available to operators licensed in the future?

Response

The licensed operator training program will be reviewed to assure that responsibilities identified in the NSO summary are covered. Applicable responsibilities not currently included in training materials will be included prior to the beginning of the next licensed training class.

l Ouestion 3 In light of the significant number of wrong unit errors at LaSalle since Unit 2 licensing, what effort did the Commonwealth Edison Company make to assure that this error was an isolated event and that there are no other baproperly hung tags currently in the field?

Response

This is the second wrong unit error at LSCS. There have been a few instances of wrong train, wrong equipment errors. We conclude this is an isolated event because the total amount of operations performed justifies treating each occurrence as an isolated event.

In support of this, during previous out of service card audits there have not been any instances where outages were improperly hung. Since this event all out of service cards in both units have been audited and found to be properly installed. The seriousness of operating wrong equipment is being reemphasized to all shift personnel.

Ouestion 4 How will the effectiveness of your corrective actions, especially those initiated to prevent wrong unit errors, be assessed?

Response

Licensee Event Reports and Deviation Reports are reviewed annually by Commonwealth Edison's Nuclear Safety Department. This review includes a breakdown of events by root causes (i.e. personnel error, wrong unit, wrong equipment, etc.).

This review is analyzed for improving and declining performance trends and will identify any significant trends involving wrong unit problems.
