Dear Mr. Denton:
This letter transmits a report of our investigation of the failure of Static "O" Ring differential pressure switch diaphragms. The attached report is the updated information referred to in Reference (a) concerning our findings and the impact on our long term corrective action plan.
Also attached to this letter is a chronological description of the events leading up to this investigation. The performance history of each of the replaced switches is still under investigation. We will continue to keep you informed as information becomes available.
